Description
Case Managers serve as an important and dynamic part of our residential treatment team by engaging and supporting patient activities. The case manager will assist patients throughout their time in the treatment program, including providing support during the initial admission and then thoroughly preparing them for discharge from Key Works.
The case manager will connect patients to internal and external services that promote their recovery. This includes but is not limited to linking patients to medical care (dental, vision, etc.), legal coordination (court dates, court appearances) and additional services. While supporting patients preparing for discharge, the case manager will assist the patients to admit into continued behavioral health providers and other services in the community and establish a clean and sober support system in the community.
Duties and Responsibilities:
  • Reliable work attendance and ability to work some evening and weekend hours
  • Demonstrated ability to establish and maintain a variety of records, organize data, prepare reports.
  • Knowledge of community resources and demonstrate ability to work cooperatively and responsively with them.
  • Ability to demonstrate diversity, equity, and inclusion in all work-related activities.
  • Ability to plan, prioritize and organize your own workload and work independently.
  • Ability to communicate effectively both in writing and orally.
  • Establish and maintain effective professional relationships and maintain strong boundaries with clients.
  • Ability to maintain confidentiality with sensitive information in accordance with HIPAA, 42 CFR and 45 CFR.
  • Applicants must successfully pass a criminal background check, pre-employment drug screen and TB test.

Requirements
Minimum Education and Experience Requirements:
  • High School diploma or equivalent required
  • 1-2 years case management experience required
  • AA Degree in SUD studies or Bachelor's Degree (preferred)
  • Must have experience working with patients with SUD and co-occurring disabilities
  • Computer and EHR literate
  • Requires a valid Washington State Driver's License
  • Must pass a driver's abstract check in accordance with site/agency insurance requirements
  • Individuals with lived experience with substance use disorders or co-occurring disorders are strongly encouraged to apply
