Description:

SUMMARY

The Case Manager provides daily client support, case management, care coordination, and individualized employment services for participants. This position supports clients in achieving treatment, recovery, placement, discharge, and employment goals while ensuring services are person-centered, recovery-oriented, properly documented, and compliant with applicable program, HHSC, HCBS-AMH, county, and organizational requirements. The role works closely with facility management, clinical staff, residential counselors, recovery managers, conservators, county representatives, employers, providers, and other stakeholders to promote continuity of care, client stability, and positive outcomes. Candidates must demonstrate maturity, good judgment, emotional stability, professionalism, confidentiality, and the ability to work independently and as part of a team. A flexible schedule, including some weekend and overnight shifts, may be required.


DUTIES AND RESPONSIBILITIES

· Provide daily support, care coordination, and case management services to clients and participants.

· Meet with each client individually to discuss Needs and Service Plan, Individual Recovery Plan, placement, discharge, employment readiness, goals, strengths, barriers, and progress.

· Update participant notes, Needs and Service Plans, Individual Recovery Plans, treatment plan documentation, and related records as required.

· Develop progress notes, summaries, and other documentation based on individual meetings, group interactions, employment services, and client progress.

· Maintain accurate and timely documentation that meets organizational, HHSC, HCBS-AMH, billing, and monitoring requirements.

· Review client referrals, complete preplacement interviews, functional capability forms, preplacement appraisals, surveys, and other intake or placement-related documentation.

· Locate appropriate placements for clients and coordinate with various case managers; state case managers, recovery managers, conservators, providers, and stakeholders as needed.

· Develop and maintain relationships with Recovery Management Entities (RMEs), Local Mental Health Authorities (LMHAs), hospitals, behavioral health providers, managed care organizations, homeless service providers, crisis centers, shelters, outpatient clinics, and other community partners to facilitate HCBS – AMH participants.

· Transport clients to scheduled court appointments or other approved appointments as needed.

· Accurately manage client appointments and communicate updates to appropriate leadership, recovery management entities, clinical staff, and support teams.

· Provide Employment Assistance and Supported Employment services to eligible HCBS-AMH participants using approved evidence-based or evidence-informed employment models.

· Assist participants with job readiness, career planning, resumes, applications, interview preparation, job searches, workplace behavior, communication skills, and steps toward obtaining employment.

· Conduct job development based on participant interests, strengths, preferences, work history, abilities, and Individual Recovery Plan goals.

· Build relationships with local employers, advocate for inclusive employment opportunities, and support competitive integrated employment at or above minimum wage.

· Provide job placement, job coaching, workplace support, and ongoing employment retention support as appropriate and authorized.

· Monitor employment satisfaction, employment progress, outcomes, barriers, and support needs; adjust support plans as needed.

· Coordinate employment services with case managers, therapists, CPST providers, peer support staff, residential counselors, recovery managers, and other interdisciplinary team members.

· Provide employment services through approved telehealth methods when appropriate and compliant with HHSC guidelines.

· Maintain strict confidentiality and handle sensitive client information and documents appropriately.

Requirements:

QUALIFICATIONS

· Must be at least 21 years of age for case management duties, unless otherwise approved by program requirements.

· College degree preferred but not required for case management duties.

· For employment specialist services, must be 18 years of age or older and meet one of the following: bachelor’s degree in rehabilitation, business, marketing, or a related human services field with one year of experience providing employment services to individuals with disabilities; associate degree in rehabilitation, business, marketing, or a related human services field with two years of experience providing employment services to individuals with disabilities; or high school diploma/GED with three years of experience providing employment services to individuals with disabilities.

· One year of related experience preferred, including understanding of psychiatric illness, treatment, and psychotropic medication.

· Previous experience with the population served or work in a similar program setting preferred.

· Valid driver’s license required.

· CPR, First Aid, and BLS certification required or must be obtained as required by the organization.

· Must pass applicable fingerprint-based criminal background checks, clearances, HHSC registry checks

· Must be free from applicable state sanctions.

· Ability to read, write, document, and communicate clearly and accurately.

· Ability to work independently, remain self-motivated, and function appropriately in an often-ambiguous environment.

· Ability to act as part of a team, provide input, report observations and concerns regarding client care, and respond appropriately in crisis situations.

· Proven ability to maintain confidentiality and appropriately handle sensitive information and documents.
