Preferred   Qualifications (education, experience, skills):
Peer   support certification
Associate degree in social work, Â psychology, criminal justice, or related field preferred

Purpose: The Reentry & Recovery Case Manager Assistant   supports individuals transitioning from incarceration, treatment programs, or   unstable living situations into stable, independent lives. This role works   closely with R&R Case Manager(s) to coordinate services, track progress, and provide hands-on support to clients navigating recovery, housing, employment, and community reintegration. This position plays a critical role   in reducing recidivism, supporting long-term recovery, and helping   individuals build stable, productive lives within their communities.Â
Essential Functions. An   individual in this role must be able to perform the following functions with   or without reasonable accommodation:
Assist case managers in developing   and implementing individualized case plansÂ
Help coordinate access to housing, healthcare, mental health services, substance use treatment, tribal resources, employment services, and other community resources as indicated in case plan. Â Â
Maintain accurate, up-to-date   client records and documentation in the Electronic Health Records System
At direction of Case Managers, conduct   client check-ins to monitor progress and identify needsÂ
Provide transportation assistance   or accompany clients to appointments when neededÂ
Support clients in completing   applications (housing, jobs, benefits, IDs, etc.)Â
Facilitate or assist with group   sessions, workshops, or life skills trainingÂ
Track and document client outcomes, including at time of discharge and six months after discharge
Build relationships with community   partners and service providersÂ
Ensure confidentiality and   compliance with organizational policies and regulations
