About This Role

OakMed is looking for passionate, dedicated, and compassionate Home Health Aides to provide meaningful, one-on-one care to patients in the comfort of their own homes during nighttime hours.

The Home Health Aide plays an important role in supporting the safety, comfort, and well-being of our patients while promoting their independence and quality of life. This position is ideal for a professional and caring individual who enjoys helping others and is comfortable working independently during overnight hours.

Our Home Health Aides work closely with the nursing and care coordination teams to provide the appropriate support and care for each patient. Essential Duties & Responsibilities

As a Home Health Aide, you will be responsible for providing compassionate, dependable, and personalized care to patients in their homes. Responsibilities may include:

  • Provide personal care assistance, including bathing, dressing, grooming, toileting, and other activities of daily living.
  • Assist patients with mobility, transfers, positioning, and other needs while following safe patient-handling practices.
  • Reposition patients as appropriate to promote comfort and help prevent skin breakdown.
  • Provide companionship and emotional support while maintaining patient dignity, privacy, and independence.
  • Prepare meals and assist with other basic nutritional needs as directed by the patient's care plan.
  • Perform light housekeeping tasks to help maintain a safe, clean, and comfortable patient environment.
  • Provide medication reminders as permitted by the patient's care plan and applicable regulations; HHA staff do not administer medications unless specifically authorized by law and company policy.
  • Observe and report changes in a patient's condition, behavior, comfort, or overall well-being to the appropriate member of the nursing or care team.
  • Assist with routine monitoring and documentation as directed by the supervising nurse and care plan.
  • Respond calmly and appropriately to urgent situations and follow established emergency procedures.
  • Maintain accurate and timely documentation of care provided, observations, and patient needs.
  • Communicate professionally and effectively with patients, family members, nurses, healthcare professionals, and the OakMed care team.
  • Follow each patient's individualized plan of care and all OakMed policies, procedures, and safety guidelines.
  • Maintain patient confidentiality and protect patient rights and dignity at all times.

Minimum Qualifications

  • High school diploma or equivalent required.
  • Valid driver's license required.
  • Reliable personal transportation required.
  • Valid vehicle registration and automobile insurance required.
  • Ability to pass a criminal background check required.
  • Must be eligible to work in the United States.
  • Basic understanding of patient care and safety protocols.
  • Ability to work independently and responsibly during nighttime hours.
  • Strong communication and interpersonal skills.
  • Ability to demonstrate professionalism, compassion, patience, and respect when working with patients and their families.

Preferred Qualifications

  • Previous experience working as a Home Health Aide, Caregiver, Personal Care Aide, or in a similar healthcare or caregiving role.
  • Previous overnight or night-shift caregiving experience.
  • CPR and First Aid certification.
  • Familiarity with electronic health record (EHR) systems or electronic documentation.
  • Experience caring for individuals with chronic illnesses, disabilities, or recovery needs.
  • Strong organizational skills and attention to detail.

Skills & Abilities

  • Successful Home Health Aides are compassionate, dependable, attentive, and comfortable providing care in a patient's home. The ability to communicate effectively with patients, families, and healthcare professionals is essential.
  • This position requires physical stamina and dexterity to safely assist with personal care, mobility, transfers, and other activities of daily living. HHA staff should be able to recognize and report changes in a patient's condition, follow care plans and safety procedures, maintain accurate documentation, and respond appropriately to urgent situations.
  • The ability to work independently during nighttime hours, while knowing when to contact the nursing or care coordination team, is essential to providing safe and high-quality patient care.
