Position Details

Position: Social Worker

Location: 261 W 35th Street, Suite 405, New York, NY 10001

Key Responsibilities

• Assist clients and families in identifying community resources and support services

• Conduct client assessments and help identify social, emotional, and environmental needs

• Collaborate with caregivers, nurses, clients, families, and community organizations

• Support care planning and client advocacy efforts

• Maintain accurate client documentation and records

• Assist with referrals to community programs, benefits, and support services

• Follow up with clients and families to ensure services and resources are meeting their needs

• Participate in client care conferences and interdisciplinary team meetings

• Help ensure compliance with agency policies and applicable regulations

• Provide compassionate support and guidance to clients and families throughout their care journey

Qualifications

• Bachelor's Degree in Social Work (BSW) required

• Recent graduates are encouraged to apply

• Strong communication and interpersonal skills
