Key Responsibilities

  • Develop and maintain linkages and referral relationships between the Department of Corrections (DOC) and local community-based organizations, primary care providers, mental health centers, substance abuse programs, and crisis response teams to ensure seamless care post-release.

  • Work with Transitional Housing Unit caseworkers, medical and mental health staff to facilitate the discharge planning process, ensuring successful community reintegration.

  • Conduct a multi-systemic needs analysis, matching offender needs with available community support services.

  • Maintain a comprehensive database of community healthcare providers to support discharge planning and re-entry efforts.

  • Ensure all Release of Information Forms are completed and that documentation is updated in appropriate systems like OPII or other approved DOC/P&P systems.

  • Monitor and evaluate discharge-planning activities, implementing strategies to reduce recidivism and improve offender reintegration.

  • Comply with HIPAA, NCCHC, and other relevant state and federal guidelines to ensure confidentiality.

  • Develop and implement strategies to improve healthcare access for discharged offenders, collaborating with CQI teams to measure and enhance the effectiveness of reentry and recidivism reduction initiatives.

  • Participate in meetings/committees as required by DOC or YesCare.

Key Qualifications

  • Bachelor’s degree in Healthcare-related field.

  • Experience in case management, discharge planning, and providing services to a physically and mentally ill population preferred. Prior corrections or re-entry experience is also desirable.

  • Required to exert up to 50 pounds of force to lift, carry, push, pull, or otherwise move objects, including the human body.

  • Required to kneel, stoop, crouch, and/or crawl occasionally.

  • Frequent walking throughout the site; to include walking up and down flights of stairs.

  • Must have and maintain sufficient strength, agility, and endurance to perform during stressful (physical, mental, and emotional) situations encountered on the job.

  • Occasional running when responding to emergencies; must be able to perform Basic Life Support (BLS).
