Job Summary
We are seeking a dedicated and compassionate Registered Nurse (RN) to join our team at Aperion Care Wesley in Chicago, Illinois. As a Registered Nurse with our organization, you will have the opportunity to provide high-quality patient care while working in a dynamic and supportive environment. This is a full-time position with a Both schedule.
Responsibilities
  • Provide direct patient care to residents with various medical conditions, including post-hospital rehabilitation and long-term skilled nursing care
  • Develop and implement individualized care plans to meet the unique needs of each resident
  • Collaborate with other healthcare professionals to ensure seamless communication and coordination of care
  • Monitor and report changes in resident condition to the medical director and other healthcare professionals
  • Document resident care and progress in accordance with regulatory requirements and organizational policies
