Job description

Crossroads at Lakewood is currently seeking an experienced QMAP with a heart of gold to join our team of professionals! We are a stand-alone residential memory care community in Lakewood, Colorado that specializes in caring for seniors with dementia. We believe in providing personalized care to our residents by learning and understanding their specific needs, embracing each of their unique personalities, and serving everyone with love, patience, kindness, dignity and respect.
Our QMAPs administer residents medications as prescribed, ensure medication stock is up to date, and assist with all levels of care and ADLs as needed. Job duties include, but are not limited to:
  • Checks and follows guidelines for distribution of medications in accordance with Federal, State and local guidelines
  • Records scheduled and as needed medications on the Electronic Medication Administration Record (EMAR)
  • Participates in medications inventories and orders medications due for re-order
  • Re-stocks med carts and keeps an accurate count of medications
  • Documents incidents and other required reporting in residents online charts
  • Maintains cleanliness of all areas, products and supplies, including med carts
  • Assists Personal Care Providers with all levels of care and ADLs
  • Answers emergency and routine calls via the residents pendant call system
  • Answers phone calls from family members, outside providers and pharmacy

Experience: 1-3 years of previous experience working in an Assisted Living community is a must; working in a Memory Care community is strongly preferred.
