Job description

Position Purpose:

The Medical Director oversees medical aspects of participant care, ensuring clinical excellence, and leading the Quality Assurance Performance Improvement (QAPI) program. Responsibilities include policy development, development and maintenance of medical policies and procedures/standards and protocols, supervision of Primary Care Providers (PCPs), and providing leadership within the community. Role is both administrative and clinical in nature.

MEDICAL DIRECTOR

Location:           Philadelphia, PA

Status:               Full time

Position Details:

Position covers 11 programs in the general Philadelphia area. Travel to sites required, typically visiting one site per day with the counties of Delaware, Philadelphia, Montgomery and Chester.  Visitation to nursing homes and work in the centers for participant care is also required regularly.

Participates in provider on-call rotation, typically two one-week assignments of one call every 3 4 months.

This position supervises our PCPs and nurse case managers. 

What you will do:

  • Participate actively in interdisciplinary team meetings, providing medical expertise and guidance.

  • Conduct intakes/assessments, develop treatment plans, and ensure preventive care for participants.

  • Discuss each participants medical conditions and prognosis with him/her and their caregiver to assist them in expressing their wishes and planning their future care.

  • Coordinate care across various settings and promote resource conservation.

  • Supervise healthcare programs and collaborate in nursing treatment plans.

  • Attend to emergency medical situations and provide timely interventions.

  • Ensure compliance with EMR optimization and documentation standards.

  • Lead advance care planning discussions and ensure culturally sensitive practices.

  • Collaborate with consultants and pharmacists to optimize participant care.

  • Oversee the QAPI program.

  • Participate in grievance procedures when complaint involved medical care.

Minimum Qualifications:

  • Current and active plenary physician license to practice in state of PA

  • CDS Certification and DEA Registration required.

  • Current BLS required.

  • Board Certification Internal Medicine or Family Practice required. Board Certification in Geriatrics preferred.

  • A minimum of one (1) year working with the frail or elderly population required.

  • Previous supervisory experience required.

  • Must have a valid drivers license, proof of insurance and use of personal vehicle required.

  • Strong communication, problem-solving, and organizational skills. Able to communicate effectively with individuals and groups representing diverse perspectives.

  • Proficiency with Microsoft Office suite

  • Ability to change and adapt to changing priorities.

  • Periodic travel within the Center's geographic region or other locations as needed
