We are hiring a Physical Therapist (PT) for a Home Health position providing skilled care to patients in their homes in the Springfield area. This position is eligible for a $10,000 first year success bonus to reward the accomplishment of key achievements during your first year!

What You’ll Do:

  • Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you

  • Perform comprehensive patient evaluations, functional mobility assessments, and develop individualized therapy plans to improve strength, balance, and independence

  • Educate and empower patients and caregivers on therapeutic exercises, injury prevention, and safe mobility strategies to support recovery and confidence at home

  • Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation

  • Work independently, but never alone

  • Travel to patient homes within assigned territory

PT Qualifications

  • Active Physical Therapist, PT license in the state you work

  • One year of experience as a Physical Therapist and the clinical expertise to deliver quality patient care

  • Current driver’s license and ability to spend ~20% of your day driving to/from patient locations
