Description
Under the direction and supervision of the Nursing Director, this position utilizes nursing and public health concepts and theories including basic epidemiology to provide selected health promotion and disease prevention services to individuals, families, and groups in a variety of settings.
Duties
  • Conduct home visits and clinic services by identifying the physical, social, and emotional health needs of individuals and families through systematic assessment; develop a comprehensive care plan based on the systematic assessment involving the patient, family, co-workers and other disciplines on the health care team.
  • Implement care plan through nursing interventions (conduct interviews and examinations, collect specimens, assess health status, change dressings, administer preventive immunizations, supervise care of home health aide and L.P.N. in home, provide instruction and health counseling).
  • Provide health education to individuals and groups.
  • Maintain records on all observations and interventions provided during home visits and clinic sessions. Document accurate and concise information in client's records and in reports to others. Complete reports and records in timely manner.
  • Ability to collect and organize data sufficient to maintain accurate and complete records, and to prepare reports in accordance with state and federal guidelines.
  • Initiates and maintains a medical record containing documentation of consent, emergency contact, screening performed, test referrals and their results, and follow-up of medical problems. Ensures that documentation is organized so that the status of the client's medical care and any follow-up efforts by staff can be determined at any point in time.
  • Provides and participates in professional education activities.
  • Completes necessary reporting requirements for all program areas.
  • All other duties as assigned
Qualifications
  • Licensed as a Registered Nurse in the State of Ohio
  • Degree in Nursing from an accredited college or university required. Associate's degree accepted, Bachelor's degree preferred
  • A minimum of two years of nursing experience or six months or more experience in Public Health preferred
  • Valid Ohio driver's license.
