Summary
SUMMARY: The Public Health Nurse (PHN) focuses on communicable disease surveillance, case management, and community-based health promotion activities to support the public health mission of the Yakama Service Unit.
  • ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:
    • Conduct case management, outreach, and contact tracing for communicable diseases such as tuberculosis, STIs, and measles.
    • Perform Directly Observed Therapy (DOT) for tuberculosis and other conditions.
    • Coordinate with local, state, and federal public health departments to ensure data reporting compliance and continuity of care.
    • Order and act upon laboratory results; notify patients of findings and provide counseling/education.
    • Develop and deliver public health education sessions for individuals, families, and community groups.
    • Perform field visits for community health follow-up and document all activities in the EHR or approved public health information systems.
  • SKILLS AND QUALIFICATIONS:
    • Demonstrated knowledge of communicable disease control and contact tracing protocols.
    • Ability to coordinate effectively with public health entities and interdisciplinary teams.
  • EDUCATION & EXPERIENCE:
    • Education: Bachelor's degree in Nursing (BSN) from a professional nursing program approved by a state accreditation agency.
    • Experience: At least one (1) year of experience in public health nursing or community outreach.
  • SUPERVISORY RESPONSIBILITY: There are no supervisory requirements for this position.
  • WORK ENVIRONMENT: Services are performed on-site at YIHC and at community outreach events within the Yakama Nation Reservation.
  • PHYSICAL DEMANDS: Must be capable of performing community field visits and participating in outreach health promotion activities.
  • ROLE and LOCATION SPECIFIC FUNCTIONS: Supports the YIHC Public Health Nursing Department. Requires coordination with external public health agencies.
  • POSITION TYPE/EXPECTED HOURS OF WORK: Up to 40 hours per week; regular clinic hours are Monday through Friday, 8:00 a.m. to 5:00 p.m. Closed federal holidays.
  • TRAVEL REQUIREMENTS: Required to drive to outreach or community activities. Travel expenses will be reimbursed.
  • CLEARANCE/LICENSE REQUIREMENTS:
    • Current, unrestricted Registered Nurse license in a U.S. jurisdiction.
    • Current Basic Life Support (BLS) certification.
    • Favorable background investigation and fingerprinting; must meet HSPD-12 requirements.
  • The nurse must be eligible to participate in the Medicare and Medicaid programs and must not be excluded, suspended, or otherwise ineligible under any federal or state healthcare program.
