Description

JOB PURPOSE:

Services provided by the person in this position should promote recovery and wellness, community integration, independent living within the community and improved quality of life. A high level of detailed electronic documentation is required and adherence to company and state guidelines is expected. Ensure delivery of wellness services of U.S. Customs and Border Protection (CBP) employees and their families by promoting and developing awareness of the program and conducting full eligibility screenings of potential clients. Provide comprehensive case management to CBP employees and their families who are seeking a better quality of life. The Case Manager/Intake Specialist is part of a multidisciplinary team who serves as the bridge between clinical services, wellness services and community resources. This position will work with clients to deliver case management in office setting and/or community-based settings.

Qualifications

ESSENTIAL JOB RESPONSIBILITIES:

  • Advocate for and facilitate client's access to wellness services and community resources and relevant benefits. Build a database of community resources. Provide and assist navigation to referrals.
  • Provides Behavioral Health and Wellness case management to Law Enforcement personnel as a part of a workforce wellness program. Provides appropriate referrals as necessary. Prepares and maintains confidential case records. Ensures confidentiality and controls access to sensitive information. Follows care follow-up procedures to ensure clients received the services they were referred to and do not have any follow up needs. Provides services via telehealth platform, as required.
  • Conduct outreach and networking activities in the community. Provide presentations at various community groups in assigned areas. Be active in and network at monthly outreach events. Promote the success and reputation of the CBP Workforce Wellness Program. Develops network of trusted contacts within organizations in which to refer and coordinate care. Consults with service providers and community partners on resource-related issues.
  • Conduct initial eligibility screening of potential clients seeking program assistance and provide general information and community referrals to clients not meeting program eligibility requirements and/or clients not scheduled with a service due to program capacity.
  • Complete Intake and Case Management for eligible clients. Evaluate individual and family needs. Complete a treatment plan with clients including wellness services and other related needs. Coordinate and monitor services, including comprehensive tracking of client activities in relation to service plan and client's needs. Document detailed case notes, daily; maintain comprehensive client files as per Policies and Procedures.
  • Other duties as assigned.

ESSENTIAL QUALIFICATIONS:

EDUCATION: Bachelor's degree in social work, Sociology, Psychology, or related field preferred OR any combination of experience and education that results in the required knowledge, skill, and abilities is required.

EXPERIENCE: 1-3 years of case management experience preferred. 2+ years in a customer service-focused environment preferred. Experience with law enforcement, veterans & their families is a strong plus.

ATTENDANCE: Must maintain regular and acceptable attendance at such level as is determined in the employer's sole discretion.

LICENSES: Driver's License with clear record. LMSW, LBSW, LMFT preferred.

OTHER: Must be available and willing to travel to various locations and with such frequency as the business need dictates.
