Equis Care Psychological Services is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our innovative team on a full-time basis. Key Responsibilities
  • Conduct comprehensive psychosocial assessments through structured interviews, observational techniques, and the careful review of medical records, identifying emotional, social, and environmental factors that might be affecting patients' recoveries.
  • Offer compassionate counseling and guidance to patients and their families, addressing their emotional well-being and helping them navigate adjustment challenges during their rehabilitation process.
  • Collaborate closely with a range of healthcare practitionersโ€”such as physicians, occupational therapists, and speech-language pathologistsโ€”to formulate individualized treatment plans that holistically integrate both mental and physical care strategies.
  • Assist patients in identifying and accessing community resources, including but not limited to support groups, vocational rehabilitation services, and housing assistance, aimed at facilitating their successful reintegration into society post-rehabilitation.
  • Provide educational sessions to patients and families about available community resources and post-rehabilitation services, empowering them to take an active role in their ongoing recovery.
  • Participate in proactive discharge planning processes, ensuring smooth transitions to home or alternative care settings, thereby supporting long-term recovery goals.
Patient Population / Client Focus

This role specifically focuses on guiding clients within the school system, addressing a spectrum of emotional and psychological needs often faced in educational environments. You will work with adolescents dealing with adjustment issues related to education, social integration, trauma, and other significant life changes, providing them support through counseling and resource navigation.

Qualifications
  • A Masterโ€™s degree in Social Work from an accredited institution, ensuring a solid foundational knowledge of clinical practices and ethics.
  • An active state license as a Licensed Clinical Social Worker (LCSW), validating your qualifications to practice independently in the state of New York.
Preferred Experience
