Adult Day Program Site Manager

St. John's Community Care is seeking a compassionate, experienced Adult Day Program Site Manager to oversee the daily operations of our Adult Day Services Program serving older adults and individuals with disabilities, including those living with Alzheimer's disease, dementia, and other cognitive impairments.

The Site Manager provides leadership for program operations, participant care, staff supervision, caregiver support, community outreach, program development, and regulatory compliance. This role works closely with the Adult Day Program Director to ensure a safe, engaging, person-centered environment that promotes dignity, independence, socialization, and quality of life for participants while supporting family caregivers.

The ideal candidate has experience in healthcare, gerontology, social services, senior services, adult day services, dementia care, nursing, case management, or program administration and demonstrates strong leadership, communication, and team-management skills.

Key Responsibilities

• Manage daily operations of the Adult Day Services Program.
• Conduct participant assessments, intake evaluations, and enrollment processes.
• Develop, coordinate, and supervise therapeutic, recreational, cognitive, and social activities.
• Provide participant-centered care and support individuals with dementia, Alzheimer's disease, cognitive impairment, physical disabilities, and age-related challenges.
• Supervise program assistants, volunteers, and support staff.
• Ensure participant safety, emergency preparedness, risk management, and compliance with organizational policies.
• Collaborate with nursing staff to review and implement individualized care plans.
• Monitor participant engagement, health status, and overall program effectiveness.
• Support family caregivers through education, communication, resources, referrals, and respite services.
• Ensure compliance with Illinois Department on Aging requirements and applicable health and safety regulations.
• Maintain nutrition and food service standards in accordance with Health Department guidelines.
• Assist with staff scheduling, workforce planning, recruitment, hiring, onboarding, training, coaching, performance management, and employee evaluations.
• Participate in community outreach, networking, and relationship-building activities to promote program services.
• Assist with quality improvement initiatives, policy development, and program growth.
• Report suspected abuse, neglect, or exploitation in accordance with Illinois mandated reporting requirements.
• Perform other duties as assigned.

Required Qualifications

• Bachelor's degree in Social Work, Nursing, Healthcare Administration, Human Services, Gerontology, Psychology, Recreation Therapy, Public Health, or a related field.
• Minimum three (3) years of leadership, supervisory, management, or program administration experience.
• Experience working with older adults, seniors, individuals with disabilities, dementia, Alzheimer's disease, or other cognitive impairments.
• Demonstrated experience supervising employees, volunteers, or multidisciplinary teams.
• Strong interpersonal, communication, conflict-resolution, and customer-service skills.
• Ability to work effectively with participants, families, caregivers, healthcare professionals, and community partners.
• Proficiency in Microsoft Office Suite (Word, Excel, Outlook) and basic computer systems.
• Valid driver's license and required automobile insurance coverage.

Preferred Qualifications

• Registered Nurse (RN), Social Worker, Licensed Clinical Social Worker (LCSW), Human Services, Gerontology, Recreation Therapy, or related professional credential.
• Adult Day Services experience.
• Dementia care certification or specialized Alzheimer's training.
• Experience with person-centered care planning.
• Experience in senior services, long-term care, assisted living, memory care, home and community-based services, case management, or healthcare program management.
• Food Safety Manager Certification.
• Knowledge of state licensing, healthcare regulations, and adult protective services requirements.
