) QUALIFICATION:
A Psychiatric Rehabilitation Services Coordinator (PRSC) shall possess a bachelor's degree in a human services field (including but not limited to: sociology, special education, rehabilitation counseling or psychology) and have a minimum of one year of supervised experience in mental health or human services.
Working Conditions and Qualifications
Ability to establish working relationships with residents, family members, staff, visitors, government agency personnel, etc. Ability to tolerate demanding, upset people in crisis situations without taking it personally. Ability to be patient, steady, cheerful and enthusiastic with the sustained motivation needed in a long-term care setting. Ability to work cooperatively and harmoniously in a problem-solving manner with a variety of personalities in a fast-paced changing environment keeping in mind the welfare of the residents.
Purpose of the Position
The primary purpose of the position is to integrate, coordinate and monitor each resident's psychiatric rehabilitation services program of a specific case load of residents in accordance with current Federal, State and local standards, guidelines and regulations, our established policies and procedures, and as may be directed by the PRSD and/or Administrator, to assure that the emotional and social needs of the resident are met and maintained on an individual basis. Generally, the responsibilities of the PRSC are:
  • To provide the resident with a stable therapeutic relationship;
  • To orient the resident to the facility;
  • To review and assist the resident in understanding the treatment plan and program

schedule;
  • To prepare and assist the resident with active participation in the treatment plan review;
  • To provide and/or coordinate the delivery of the psychiatric rehabilitation services

programs; and
  • To monitor the resident in the areas of self-directed care and for overall compliance

with the treatment plan.
DUTIES AND RESPONSIBILITIES:
Administrative Functions
  • Assist in the planning, developing, organizing, implementing, evaluating and directing the psychiatric rehabilitation services programs of this facility.
  • Assist in developing and implementing policies and procedures for the identification of related social and emotional needs of the resident.
  • Perform administrative requirements, such as completing necessary forms, reports, etc., and submitting such to the PRSD as required.
  • Assist in arranging services and transportation to other appointments when necessary.
  • Provide consultation to members of our staff, community agencies, etc., in efforts to solve the needs and problems of the resident through the development of specialized psychiatric rehabilitation services programs.
  • Review and may evaluate the Rehabilitation Assistant's performance and make recommendations to the PRSD.
  • Coordinate psychiatric rehabilitation services activities with other departments as necessary.
  • Work with the facility's consultants as necessary and implement recommended changes as required.
  • Make written and oral reports/recommendations to the PRSD concerning the operation of the psychiatric rehabilitation department..
  • Participate in surveys (inspections) made by authorized government agencies.
  • Be prepared to handle emergencies as they come up (i.e., rescheduling work assignments and work schedules, etc.).

Psychiatric Rehabilitation Functions
  • Know the psychiatric rehabilitation program and the regulations governing psychiatric rehabilitation services and resident rights.
  • Initiate and oversee the assessment process of all new and re-admitted residents in the assigned case load. Complete necessary assessments..
  • Facilitate Care Plan conferences for each resident in the assigned case load, prioritizing needs, writing goal statements and discharge plans with the interdisciplinary team. Care plans may be in response to significant change, initial for new admissions, quarterly and annual care plan conference.
  • As part of the psychiatric rehabilitation services treatment program, provide 1:1 counseling groups, training and behavioral intervention to develop resident skills in self-maintenance, social functioning, community living skills, symptom management and substance abuse and vocational rehabilitation.
  • To be knowledgeable of the problems, behaviors and care plan of each resident in the caseload.
  • Write progress notes on objectives, monthly notes, quarterly notes, review IDT notes, conduct chart audits and assure that all progress notes charted are informative and descriptive of the services provided and of the resident's response to the service.
  • May schedule and assign treatment programs for the Rehabilitation Aid, the IDT and Day Programs.
  • Works with the PRSD in developing positive working relationships with the IDT.
  • Assist in solving social and practical environmental problems including seeking financial assistance, discharge planning (including collaboration with community agencies), and referrals to other community agencies when specialized assistance is required.
  • Evaluate social and family information and assist in determining plans for additional social service needs.
  • Interpret social, psychological, and emotional needs of the resident/family to the IDT.
  • Provide consultation to members of our staff, community agencies, etc., in efforts to meet the needs of the resident through the development of psychiatric rehabilitation services programs.
  • Involve the resident/family in planning objectives and goals for the resident.
  • Meet with the interdisciplinary team in planning psychiatric rehabilitation services programs and activities.
  • Follow established fire, disaster, safety, infection control, and evacuation policies and procedures.
  • Other duties as deemed necessary and appropriate, or as may be directed by the PRSD or Administrator.
