Acute Care Case Manager - Care Coordination
Schedule
Weekend rotation required; specific weekend shifts vary based on unit needs.
Requirements
- Minimum 2 years of acute medical care management experience with care coordination.
- Travel experience required (no first-time travelers).
- California license required (pending accepted during process but must be active on start date).
- Epic experience is required.
Roles & Responsibilities
- Conduct patient assessments and coordinate care for adults and geriatrics.
- Develop and implement discharge plans and participate in family meetings.
- Arrange durable medical equipment (DME), skilled nursing facility (SNF) placement, and transport as needed.
- Manage complex psychosocial needs, including homelessness, mental health, and substance use.
- Ensure compliance with CMS, DRG, and JCAHO standards, along with HIPAA and OSHA regulations.
Compliance / Notes
- Orientation consists of 40 hours.
- Return-to-office restriction is 7 days.
- Must commit to working 3 of the 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day).
- Dress code includes turquoise scrubs or business casual attire.
