Overview: The Case Manager II will act as a member of a Community Public Health Team (CPHT) to provide case management services, follow-ups, referrals, advocacy, and wraparound services to clients.
Duties and Responsibilities (other duties as assigned):Reporting to the Program Manager - Prevention and Outreach Services, the case manager II will develop in partnership with community members, and multi-disciplinary team partners, client centered, strength-based case management plan. The case manager II will meet with program clients referred through the household needs assessment to assist in removing barriers to meet case management goals and linking clients to on and offsite resources. Liaise: Work in partnership and coordination with CPHT partner agency, Northeast Valley Health Corporation (NEVHC) and designated Case Managers Develop: comprehensive needs assessments, coordination of services, and provides education. Will develop goals based upon client stated needs and identification of problem(s), will evaluate and monitor progress. Client Services: Will provide systems navigation, linkages to referrals for health care, mental health services or other social services, provide collateral resources, and monitor client progress. Assists Clinicians in coordinating additional resources for clients as needed. Will ensure client's basic needs are met, outside of mental and physical health, providing a continuum of care and wrap around services. Provides Assertive Outreach. Data Collection and Documentation - Collect and analyze health data. Maintain accurate documentation, monthly reports, inputting data into the data management systems. Completes required reports for funders, responsible for meeting grant milestones. Training and Workshops: Participate in weekly multidisciplinary meetings to build capacity and develop sustainability plans. Attends staff meetings, participates in training of staff, volunteers, and the community. Performs other duties as assigned.
Qualifications:
Has experience working with adults, families and/or children in the community or experience in community organizing or in a social service setting is preferred. Must be a highly organized individual able to multi-task, track and document case management of clients. Computer competency, including Word, Excel, Outlook and PowerPoint. Bilingual English/Spanish preferred.
Knowledge, Specialized Skills, and Abilities:
Must demonstrate the ability to use communication skills that successfully convey and obtain accurate and complete information in a respectful and caring manner. Have a basic knowledge of body language and non-verbal communication. Understand and demonstrate active listening skills. Must be a critical thinker, reasoning, organizing and analyzing information so that problems are accurately understood and solutions are congruent with client's capacities and outcome orientated. Capacity to negotiate and collaboration to insure the client's successful receipt of services. Demonstrate the skill of advocating for the needs and rights of people. Must show a passion and sensitivity for trauma work. Demonstrate experience to maintain cooperative working relationships with others; be team oriented; coordinate/collaborate with professionals from multiple agencies whilst maintaining appropriate boundaries and client confidentiality.
Pay, Benefits, & Work Schedule
This position is employed through The University Corporation (TUC).
The anticipated hiring range: $24.04 - $27.00 per hour
