Job description

WHAT YOU'LL DO:

Under the direction and guidance of the Program Manager, you will be responsible for providing comprehensive, strength-based case management services to homeless and/or marginally housed transitional aged youth (ages 18-24). You will conduct a comprehensive evaluation of bio-psychosocial, emotional, educational, vocational and functional needs of young people; develop an individualized service plan in coordination with Larkin Street Youth Services continuum of care as well as leverages relevant community resources as needed.  The Case Manager II leads collaborative service planning and must have a thorough understanding of the stages of change and development. The Case Manager II must be comfortable providing services to clients outside of normal office settings, including but not limited to emergency shelter environments, congregate living housing placements, scattered site housing, and single room occupancy (SRO) placements throughout the city of San Francisco.

This is a full-time position, eligible for full benefits.

Your responsibilities:

  • Conduct intake and assessment; direct service linkage and referral; case planning and contracting; case coordination and advocacy; follow up and after care.
  • Develops and maintains strong relationships with clients.
  • Provide crisis prevention, intervention, and safety planning services, along with stress management and life skills development.
  • Assist young people in applying for and obtaining benefits and housing.
  • Assist in providing client resources of outside agencies and supportive services that promote the health and wellness of youth.
  • Work closely with community behavioral health providers and internal service teams to ensure coordinated and effective services.
  • Maintain accurate detailed observation and progress records and completes necessary electronic data entry into agency and city databases.  Identifies trends in data to assist in collaborative service planning.
  • Adhere to laws regarding confidentiality (HIPAA) and reporting requirements and maintains necessary compliance documentation.
  • Collaborate with fellow staff members and established community partners, including property managers and landlords.
  • Facilitate transportation and provides accompaniment to client appointments in the community.
  • Complete safety and wellness checks regularly in residential units.
  • Teach life skills (e.g., cooking, cleaning, budgeting).
  • Participate in organizational meetings (e.g., all-staff, CQI, Client Practice, etc.)
  • Provide services when and where clients need them, including evenings and weekends when program needs dictate.
  • Support program services within the milieu.  For example, crisis intervention or helping to maintain the safety of the program.
  • Other general duties contributing towards the care and welfare of the clients we serve.

LOCATIONS

This is a pooled job posting. You could interview at any of the following programs:

  • Residential Programs
    • Location: Any of our housing programs located throughout San Francisco. 
      • This may include sites in the Tenderloin neighborhood.
    • Hours: 9AM to 5PM OR 10AM to 6PM, depending on site needs.
    • Working Days: Tuesday through Saturday OR Sunday through Thursday. 
      • **working schedule includes weekends.
  • Rapid Re-Housing Programs
    • Location: Based out of the San Francisco office, the RRH Case Manager II completes home visits and case management sessions with their caseload at various locations in and out of the office.   
      • This may include sites throughout the Bay Area generally within a 50 mile radius of San Francisco and accessible by public transportation.
    • Hours: 9AM to 5PM OR 10AM to 6PM, depending on site needs.
    • Working Days: Tuesday through Saturday OR Sunday through Thursday.
      • **working schedule includes weekends.
  • Drop-In Center
    • Location: The Engagement and Community Center (ECC), 134 Golden Gate Ave, San Francisco, CA
    • Hours: 9AM to 5PM OR 8AM to 4PM, depending on site needs. 
    • Working Days: Monday through Friday.
