Schedule: 8:30 AM to 4:30 PM, Monday - Friday
Program is based at our new Adult Day Program in Thousand Oaks!
Momentum is hiring a Program Instructor to support individuals with daily living needs in a classroom setting. As a Program Instructor, you will be responsible for providing instruction to our people supported in computer technology, painting, gaming programs, and other activities; all while creating an environment where our people feel welcomed, invited, and supported in their personal and group exploration. Program Instructor Essential Duties:
  • Promotes a friendly, comfortable, and safe environment
  • Respects and promotes the right of individuals
  • Develops lesson plans for individuals consistent with Individual Service Plan (ISP) objectives
  • Manages day-to-day classroom activities consistent with ISP objectives
  • Tracks and encourages program attendance
  • Ensures that person(s) supported are clean and safe from harm or injury
  • Assists people attending the program with communication, feeding, mobility, toileting, hygiene, and other activities of daily living
  • Documents and completes person(s) supported notes, ensuring they are accurate and consistent with individuals behavior
  • Implements quality behavior intervention plans
  • Assists person(s) supported with community-based activities consistent with ISP objectives
  • Follows and implements company policies
  • Attends required in-service trainings
  • Performs other duties as assigned to meet department and/or business needs

Program Instructor Qualifications:
  • Must be 18 years of age or older
  • High school diploma or GED (or substitute experience)
  • At least one (1) year of experience in care and treatment of individuals with developmental disabilities; preferred
  • Have working knowledge of computers and adaptive technology; preferred
  • Website design and maintenance; preferred
  • Ability to troubleshoot problems with computers, adaptive equipment, and software
  • CPR and First-Aid certified
  • Participate in pool therapy and aquatic activities
  • Read, write, comprehend, and communicate in English well enough to perform essential duties and responsibilities
  • Ability to maintain confidentiality
  • Communicate and work professionally and cooperatively with others
  • Successfully pass all post offer, pre-employment screening requirements, including criminal background check, drug screening, health screening, TB screening and provide proof of eligibility to work in the United States
  • Ability to run, push, pull, bend, squat, kneel, and lift up to fifty (50) pounds
  • Valid and current proof of legal right to drive in California; required
