About the Program:

Health Homes is an outreach program which engages clients (including pediatrics) in the community with health and social services to best meet their needs. Health Home (HH) Services are intensive services that coordinate care across several domains. The purpose is to coordinate clinical and social service expertise for high risk clients with complex chronic conditions, mental health challenges, substance use disorder issues, and/or long term service needs and supports.

Duties & Responsibilities:

    • Worksas a member of a multidisciplinary team.

    • Providesadministrative support and supervision to team members including approval of timesheets, leave requests, supply requests, mileage and expense reporting, completion ofperformance reviews as scheduled,disciplinary incidents,and monitoring compliance of training requirements.

    • Screensreferrals for eligibility and appropriateness. Assign accepted clientstoappropriateteam members.

    • Notifiesreferral sources of acceptance into the program or inform them why theclient could not be accepted.Notify them of any delays in starting services andapproximate time frames.Maintainawaitinglist,when needed.

    • Ensuresthat all clients have a Health Action Plan and oversee development and needed revisions to address clients' changing needs.

    • Providesregular and supportive supervision toteam, ensuring that they fullyunderstandprogramobjectives,case note documentation process,program requirements, bestpractices,andagencyandprogrampoliciesandprocedures.

      • Monitorscontract changes and negotiations with all signed contracts and CCO's.

      • Attends all contractual meetings as arepresentative ofSunriseHealth Homes Program.

      • Facilitatesall staff meetings and team lead meetingsandcompletedone-on-onemeetings with Care Coordinators for caseload reviews, progress checks,to include routine coaching and support to driveprogramdivisional and agency goals andobjectives.

      • Processesclients that are opting out of the Health Homes program.

      • Completion and oversightof auditsand reviews with all contracts.

      • Manage due diligence completion to ensure accuracy and communicate changes withinsurances.

      • Identifiesand completion ofreinitiated services forreturning clients to contracted CCO's.

      • Overseesall new hire interviews,candidateselection,and new hiretraining and orientation.

      • Manages all Release of Informationand Records Requests.

      • Overseesandmanagesprogramofficespaceswherestaffarelocated-inEverett,MtVernon, and Bellingham.Reportsany maintenance problems.Worksin partnership with leadersof co-located programstoensureeffectivelyshared resourcesandspaces.

      • Actively markets and promotesawareness of theHealth Homesprogram.Overseesteammarketingandnetworkingactivities.

      • Effectively managesprogram budget - ensure effective tracking systems are inplace.Workclosely withaccountingdepartment,asneeded.

      • Seekconsultationandfeedbackfromdepartment head, executive management,programstaff,clients,familymembers,andresidentialproviders.

      • Developssatisfactionsurveysto becompleted byclients,referralsourcesand residential providers. Collect, compile, and report datafromreturnedsurveys.

      • Developsappropriateshorttermand long-termobjectives, strategies, standards, and processes for Health Homes operations with the goal of continuous measurable improvement.

Education/Experience:

    • Education:B.A. of Social Services, or relatedfield.

    • Experience:Atleast twoyearssupervisory / management experiencerequired.

    • Other Considerations:

    • DSHSbackgroundclearancerequired.

      • CurrentvalidWashingtonStatedriver'slicense,acceptabledrivingrecord,andproofof insurance.

    • Skill using Microsoft Office products includingWord, InternetExplorerorotherbrowserandOutlook.

      • Strongverbalandwrittencommunicationskills.

      • Flexible.Demonstratedability to exercise sound judgment in challengingdynamicsituationsincludingbehavioralissues.

      • Skillinreading,synthesizing,andactinguponmultifacetedinformation.

Schedule:

  • Monday - Friday 9 am - 5 pm

Compensation:

  • $34.62 - $40.38/hr
  • Wages are determined by applicable industry experience, education, and certifications/licensure.
  • Benefits:

    • Paid vacation - Earned at .04 per hour worked for hourly employees, and 6.67 per month for salary (equates to about 2 weeks per year for full-time employees)
