DEPARTMENT: AdministrationJOB TITLE: Director - Transformation ProgramSHIFT: FTE 1.0 (80 hours bi-weekly)Summary/Objective

The Transformation Program Director is responsible for leading and facilitating performance improvement, operational excellence, and strategic project initiatives that support Regional Health Transformation Program (RHTP) goals and organizational priorities. This position employs Lean, Six Sigma, change management, and project management methodologies to drive measurable improvements in quality, safety, patient outcomes, operational efficiency, care delivery, and value-based care performance.

The Transformation Program Director partners with clinical, operational, and executive leaders to identify opportunities for improvement, lead cross-functional teams, manage complex projects, analyze performance data, and implement sustainable solutions that advance organizational and community health objectives. The role serves as an internal consultant, coach, and facilitator, fostering a culture of continuous improvement and accountability across the organization.

Essential FunctionsProcess Improvement Leadership
  • Lead and facilitate process improvement initiatives using Lean, Six Sigma, Kaizen, A3, PDCA, and other performance improvement methodologies.
  • Identify opportunities to improve quality, safety, patient experience, operational effectiveness, access, and financial performance.
  • Conduct workflow analysis, process mapping, root cause analysis, and value stream mapping to identify inefficiencies and waste.
  • Develop and implement sustainable solutions that drive measurable performance improvement.
  • Facilitate rapid improvement events, Lean workshops, and multidisciplinary improvement teams.
  • Monitor implementation progress and ensure achievement of targeted outcomes.
RHTP Program Support and Management
  • Lead and coordinate projects supporting Regional Health Transformation Program (RHTP) initiatives and related population health strategies, assuring compliance with milestone dates.
  • Collaborate with internal and external stakeholders to ensure successful implementation of RHTP goals, milestones, deliverables, and reporting requirements.
  • Support development, execution, and evaluation of initiatives focused on prevention, chronic disease management, healthcare access, care coordination, health equity, and community health improvement.
  • Track project performance metrics and outcomes to demonstrate program impact and compliance with funding or regulatory expectations.
  • Identify risks, barriers, and opportunities impacting project success and develop mitigation strategies.
Project Management
  • Manage multiple complex projects simultaneously from initiation through implementation and evaluation.
  • Develop project charters, scopes, timelines, work plans, risk assessments, communication plans, and status reports.
  • Establish project goals, milestones, deliverables, and success measures in collaboration with stakeholders.
  • Facilitate project meetings and maintain accountability for action items and project outcomes.
  • Ensure projects are completed on time, within scope, and aligned with strategic priorities.
  • Provide regular updates to leadership regarding project status, achievements, risks, and resource requirements.
Data Analysis and Performance Measurement
  • Collect, analyze, and interpret clinical, operational, financial, and population health data to support decision-making.
  • Develop dashboards, scorecards, and/or reports to monitor project outcomes and organizational performance.
  • Utilize data to identify trends, gaps in care, process variation, and improvement opportunities.

Change Management and Stakeholder Engagement
  • Partner with physicians, providers, nursing leaders, operational teams, population health, and community partners to support change initiatives.
  • Facilitate stakeholder engagement and build consensus around improvement priorities and solutions.
  • Coach leaders and teams on continuous improvement principles, problem-solving methodologies, and change management practices.
  • Support adoption of new workflows, technologies, and care models through effective communication and training strategies.

QualificationsExperience
  • Minimum of three to five years of experience in healthcare process improvement, project management, quality improvement, operational excellence, or population health.
  • Demonstrated experience leading crossโ€‘functional healthcare improvement initiatives.
  • Knowledge of Lean, Six Sigma, performance improvement, project management, and change management methodologies.
  • Experience supporting Regional Health Transformation Programs (RHTP), Delivery System Reform Incentive Payment (DSRIP) programs, or similar healthcare transformation initiatives.
  • Knowledge of healthcare quality metrics, accreditation standards, and regulatory requirements.
Skills/Competencies
  • Strong communication skills.

  • Ability to multitask and problem solve.
  • Proficiency with Microsoft Office applications, project management tools, and data reporting systems.
Education Required
