Job Description:

Purpose:
To provide support and assistance to individuals with intellectual and developmental disabilities to enable them to live valued lives in the community.
Qualifications:
  • Must successfully complete the required pre-employment Background and Drug Screenings
  • Must possess a valid Driver's License with all necessary endorsements and have a minimum of 3 years of driving experience.
  • Must possess advanced computer skills
  • Must possess a High School Diploma or equivalent, bachelor's degree preferred
  • Experience working in the field of intellectual/developmental disabilities preferred
  • Minimum one year of experience in writing program plans, case notes or other relevant documentation required

Essential Functions:
  • Responsible for the development, implementation and coordination of individual's program plans
  • Develop, implement and monitor individualized training programs to meet identified priority training needs
  • Ensures all plans are person centered and reflect the individual's hopes and dreams through active participation wherever possible
  • Ensure all documentation is accurate and complete according to regulatory requirements
  • Communicate with all team members, including family, guardians and support coordinators to ensure that the individual's needs are being addressed
  • Performs other duties as assigned
