Job Summary
As a Home Health Aide, you will work one-on-one with members in their homes, following an individualized care plan. This role is ideal for someone with a big heart who values building genuine relationships while providing attentive, personalized care. You should be organized, detail-oriented, and committed to creating a safe and healthy environment that meets each member’s needs. Strong communication skills are important. Prior Home Health experience is a plus.
Responsibilities
  • Assist members with ADLs, including bathing, dressing, toileting, grooming, and safe mobility
  • Support meal preparation and encourage healthy eating routines
  • Maintain a clean, safe home environment, including changing bed linens, doing laundry, and light housekeeping
  • Keep accurate records of care, observations, and any changes in condition
  • Communicate updates or concerns to the supervisor
  • Provide companionship, conversation, or reading to help keep members mentally engaged
