We are currently seeking a Health Home Care Manager Team Lead to support our growing Health Home program in Onondaga County.
Position Overview
As a Health Home Care Manager Team Lead, you will provide leadership, coaching, and support to a team of Care Managers while helping ensure quality services for the individuals we serve. This role combines direct member support with staff mentorship, training, and quality oversight.

What You'll Do
  • Coach and support Care Managers to ensure high-quality service delivery.
  • Conduct monthly field observations and appointment shadowing to provide ongoing feedback and professional development.
  • Mentor and support newly hired Care Managers during onboarding.
  • Facilitate team meetings and training sessions focused on best practices, policies, and program standards.
  • Attend Health Home Hub meetings and share updates with staff and leadership.
  • Maintain a small caseload of Health Home members.
  • Assist with member enrollment activities as needed.

Qualifications
  • Bachelor's degree in a Health or Human Services field and at least 2 years of relevant experience preferred; OR
  • Registered Nurse (RN) with at least 2 years of relevant experience; OR
  • Master's degree in a related field with at least 1 year of relevant experience.
  • Previous Health Home, care management, case management, or care coordination experience preferred.
  • Strong leadership, communication, organizational, and problem-solving skills.
  • Ability to work independently while supporting a collaborative team environment.
  • Valid New York State driver's license and reliable transportation required.
