Position: Care Coordinator

Pay Rate: $14.00/hr (Paid Weekly)
Work Location: Optum Ridge - Little Rd, New Port Richey Fl 34654
Schedule: Monday – Friday, 8:00 AM – 5:00 PM

Position Overview

We are seeking a detail-oriented Care Coordinator to join a busy Optum healthcare team. In this role, you will support patient care by processing referrals, coordinating communication with specialists and imaging centers, and ensuring timely documentation. This position plays a key role in maintaining efficient patient workflows and delivering exceptional customer service.

Key Responsibilities

  • Process patient referrals accurately and efficiently.
  • Answer incoming phone calls and assist patients, providers, and external offices.
  • Obtain consult notes, imaging reports, and other required documentation from specialists and imaging centers.
  • Coordinate communication between providers, patients, and outside healthcare facilities.
  • Maintain accurate patient records and documentation within the electronic medical record (EMR) system.
  • Ensure referrals are completed in a timely manner to support continuity of care.
  • Provide outstanding customer service while maintaining patient confidentiality.
  • Perform additional administrative and care coordination duties as assigned.

Required Qualifications

  • High School Diploma or GED.
  • Previous experience in a healthcare or medical office environment.
  • Strong organizational, communication, and multitasking skills.
  • Ability to work efficiently in a fast-paced environment.
  • Proficiency with Microsoft Office and electronic medical record (EMR) systems.

Preferred Qualifications

  • Previous referral coordination or referral processing experience.
  • Experience working with specialists, imaging centers, or provider offices.

Team Environment

The Care Coordinator will join a collaborative team of approximately 20 staff members, including 4 healthcare providers, dedicated to delivering high-quality, patient-centered care.
