PRN Weekend Registered Nurse | Home Health

Elara Caring is seeking a compassionate and dependable PRN Weekend Home Health Registered Nurse to provide skilled, one-on-one care to patients in their homes. This is a great opportunity to maintain flexibility while using your clinical skills to help patients recover, manage chronic conditions, and remain safe and independent at home.

We offer flexible weekend scheduling options, including Saturdays, Sundays, every weekend, or every other weekend. We're seeking an RN who can select a schedule that works for them and maintain consistent availability.

As a PRN Weekend Home Health RN, you will:

  • Complete scheduled skilled nursing visits in patients' homes

  • Perform comprehensive nursing assessments and reassessments

  • Provide care according to physician orders and each patient's Plan of Care

  • Monitor patients' conditions, symptoms, medications, and responses to treatment

  • Perform skilled treatments and procedures within your scope of practice

  • Identify and communicate changes in condition, safety concerns, or emerging clinical needs

  • Educate patients and caregivers about diagnoses, medications, treatments, and self-care

  • Coordinate with physicians, clinical leadership, on-call staff, and other members of the care team

  • Respond appropriately to patient needs and clinical concerns during weekend coverage

  • Take proactive steps to reduce avoidable hospitalizations and emergency department visits

  • Maintain accurate, timely, and complete clinical documentation

  • Follow all agency, safety, regulatory, and HIPAA requirements

  • Graduate of an accredited nursing program

  • Current, unrestricted Registered Nurse license in the applicable state

  • At least one year of nursing experience in a clinical care setting

  • Consistent weekend availability

  • Ability to work independently and make sound clinical decisions in the field

  • Strong assessment, communication, organization, and documentation skills

  • Dependable vehicle, valid driver's license, and current automobile insurance

  • Willingness and ability to travel throughout the assigned coverage territory

  • Physical Requirements: Sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs.
Preferred Experience
  • Previous home health or hospice nursing experience

  • Homecare Homebase experience

  • Bachelor of Science in Nursing
