While the summer months are often considered slower in Phoenix, our Skilled Nursing Facility and outpatient clinic continue to experience strong demand - we are fully booked with an active patient waitlist.

This weekend PRN Physical Therapist will work inpatient Med A evals for our skilled nursing facility. 

Join an amazing in-house team dedicated to enriching the lives of seniors through compassionate care and innovative programs. As a Physical Therapist, you'll provide exceptional services within Assisted Living, Skilled Nursing, and Memory Care residents. This role will foster a positive, supportive environment for residents, families, and team members on a PRN basis.

Pay Rate: $42 - $65 per hour

What you'll do:

    Responsible for the maintenance and accuracy of records in regard to patient care for all patients on personal case load.
    Responsible for the correct reporting of billing information of Physical Therapy in regard to all patients on personal case load.
    Provide direct patient care as prescribed, including evaluation and treatment planning.
    Responsible for the reporting and interpretation of treatment results.
    Participate in development and presentation of in-services for the facility as requested.
    Supervise support personnel in immediate work areas as appropriate.
    Perform clinical functions: screens, evaluations, development of treatment plans, and delivery of treatment in accordance with facility, Medicare, and Facility's policies and procedures.
    Assist and assess transfers, gait and balance as required by functional mobility of patient level of assistance from minimum to maximum.
    Manual manipulation, including pushing and/or pulling of patient body parts and limbs.
    Participate in meetings regarding patient care as appropriate.
    Assist when appropriate with any community education and public relations activities of the facility.
    Maintains a high standard of resident care.  
    Responds immediately to all crisis and/or calls for assistance.
    Actively participates in in-services, department meetings and continuing education.
    Provide education to patient's family as well as staff on the specific treatment plan as well as therapy goals for the patient.
    Responsible for the coordination and cooperation with other staff members in carrying out the treatment plan as established.

Who you are:

A minimum of Doctorate of Physical Therapy. Sedentary tasks related to job - approximately 10%
Physical tasks as listed - approximately 90%. CPR certified. Licensed by the State in the designated geographic area in their respective therapy profession.
