Home Care RN Case Manager - Full Time

Now hiring an experienced Full-Time Home Care RN (Registered Nurse) Case Manager for BrightStar Home Care! Join our compassionate team at BrightStar Care of Maple Grove & Andover and take your career to the next level!

Work Schedule: Monday-Friday

Pay Rate: $45 per hour, $200/Week on-call pay

Responsibilities of an RN Case Manager:

  • No OASIS work!
  • Provides care as defined within the scope of the state's nurse practice act.
  • Makes the initial evaluation visit and regularly reevaluates the client's nursing needs.
  • Prepares clinical and progress notes and summaries of care.
  • Serve as backup for Critical Care Clients, In-School Staffing & PDN
  • Assures proper maintenance of clinical records in compliance with local, state, and federal laws.
  • Helps educate and coach care-giving staff and assists with new hire clinical training, as needed.
  • Coordinates cares with Hospice Agency Nurses, Medicare-home health agencies, and other providers as needed.

RN Case Manager Requirements:

  • Current unrestricted RN license in Minnesota
  • Minimum two (2) years of clinical experience with one (1) year of experience in a home care agency, primary care clinic, or health facility, preferred.
  • Critical Care Experience (1-2 years) preferred
  • Active CPR Certification
  • Negative TB skin test or QuantiFERON-TB Gold (QFT) blood test (completed within the last 90 days)
  • Reliable car & Auto Insurance
  • Valid Driver's License
  • Clean background and Criminal Record
  • Understands regulatory compliance with state and federal regulations.
  • Ability to work on-call rotation with compensation
