PRN Registered Nurse, Home Health

Would you like to join a rapidly growing, locally owned company committed to providing exceptional patient care? James River Home Health is actively seeking a compassionate and dedicated PRN Home Health RN Case Manager to support our patients in the Prince William territory. In this role, you will manage a caseload of patients in their homes while collaborating with the interdisciplinary team to ensure a seamless, high-quality care experience.


Compensation: $43.27 - $52.88 per hour for orientation, training, required meetings, and in-service education


This is a PRN position with eligible compensation on a per-visit basis. Visit rates vary by visit type and will be reviewed with candidates during the interview process.

A brief overview of Responsibilities:

  • Provides skilled nursing care to homebound patients in accordance with the established Plan of Care, developing and implementing individualized treatment plans while monitoring and reporting changes in condition.

  • Coordinates and manages multidisciplinary services to ensure continuity of care, including admissions, transfers, and discharges, and collaborates closely with the interdisciplinary team.

  • Educates patients and families on health maintenance and disease prevention, completes timely electronic documentation in Homecare Homebase, and offers guidance and emotional support throughout the care journey.


Qualifications:

  • Must be a Registered Nurse with a current license to practice nursing in the Commonwealth of Virginia

  • At least one year of home health experience preferred

  • Graduate of an accredited nursing school.

  • Must have a valid driver's license and a reliable car available during work hours.

  • OASIS and HCHB experience PLUS!
