About the Opportunity
NurseAmerica is building a talent pool of qualified Home Health Registered Nurses (RNs) for current and upcoming opportunities with healthcare organizations across Oklahoma.
We are seeking compassionate, dependable, and independent nurses who are comfortable providing one-on-one skilled nursing care in patients' homes and collaborating with patients, families, physicians, and multidisciplinary healthcare teams.
Position Summary
The Home Health RN provides skilled nursing care to patients in their homes based on individualized plans of care and physician orders. The RN performs comprehensive assessments, administers medications and treatments, monitors patient conditions, provides education, coordinates care, and communicates changes in patient status to the appropriate healthcare providers.
The ideal candidate is comfortable working independently, managing a daily schedule of home visits, documenting care accurately, and providing compassionate patient-centered care.
Key Responsibilities
  • Provide skilled nursing care to patients in their homes.
  • Perform comprehensive patient assessments and reassessments.
  • Develop, implement, and evaluate individualized plans of care.
  • Monitor patients' conditions and communicate significant changes to physicians and healthcare providers.
  • Administer medications, injections, IV therapies, and other treatments as ordered.
  • Perform wound care, dressing changes, ostomy care, catheter care, and other skilled nursing procedures as applicable.
  • Obtain vital signs and perform appropriate clinical assessments.
  • Provide patient and family education regarding disease management, medications, treatments, and self-care.
  • Assist with care coordination and discharge planning.
  • Collaborate with physicians, therapists, social workers, aides, and other members of the healthcare team.
  • Maintain accurate, timely, and complete clinical documentation.
  • Follow applicable home health policies, procedures, infection-control standards, and regulatory requirements.
  • Respond appropriately to patient and family concerns and communicate follow-up needs.
  • Manage daily visits and prioritize patient needs effectively.
  • Participate in quality improvement and continuing education activities.
  • Perform additional nursing responsibilities as assigned.
Qualifications
  • Current Registered Nurse (RN) license in Oklahoma or applicable Compact RN license.
  • Graduate of an accredited nursing program; ADN or BSN accepted.
  • 1-2+ years of RN experience preferred, depending on assignment.
  • Previous Med/Surg or acute-care experience preferred.
  • Previous Home Health experience is preferred
  • Current BLS/CPR certification.
  • Valid Oklahoma driver's license.
  • Reliable personal transportation.
  • Valid automobile insurance meeting applicable requirements.
  • Ability to travel to patients' homes within the assigned service area.
  • Strong communication, assessment, organization, and documentation skills.
  • Ability to work independently and manage a patient visit schedule effectively.
