Position Overview

The Home Health Patient Coordinator plays a key role in ensuring smooth, timely, and compassionate coordination of patient care. This position supports both patients and clinical staff by managing scheduling, processing referrals, and maintaining accurate communication and documentation.


Responsibilities

  • Schedule patient visits and coordinate assignments with clinicians.
  • Work closely with clinical staff to ensure efficient admission and onboarding of new referrals.
  • Answer incoming calls with professionalism while maintaining strict patient confidentiality.
  • Manage referral documentation, data entry, and timely communication of medical information to physicians and therapists.
  • Assist with outreach to patients, caregivers, providers, and facilities to support continuity of care.


Qualifications

  • Experience in patient intake, medical office administration, or related healthcare roles.
  • Background in insurance authorization processing preferred.
  • Strong organizational skills with the ability to multitask and prioritize in a fast‑paced environment.
  • Professional, people‑oriented communicator—comfortable in person, by phone, and via email.
  • Knowledge of medical terminology, anatomy, and physiology is ideal; training available for the right candidate.
  • Proficiency with Microsoft Office, including Excel and Word.
  • Ability to work onsite at our Appleton office from 8am–5pm.
