Job description

AdventHealth Primary Care Network is seeking an Advanced Practice Provider to join our team. This role offers a supportive practice environment, broad clinical exposure, and the opportunity to care for patients across the lifespan while maintaining a sustainable work-life balance. This model focuses on follow-up care of previously hospitalized patients to minimize readmission, ensuring patients are receiving care throughout their recovery. We place a strong emphasis on patient-centered care within the network through patient engagement, digital health tools, cultural sensitivity, and accessible locations.

Key Responsibilities
• Provide comprehensive, patient-centered care to patients of all ages
• Deliver independent and collaborative medical management of patient concerns and conditions
• Order and interpret diagnostic testing, including labs and imaging
• Monitor treatment plans and coordinate referrals to physicians or specialists as needed
• Collaborate closely with physicians and clinical staff to ensure continuity and consistency of care
• Deliver care with a professional, compassionate, and patient-focused approach
Education & Experience
• Master's degree in nursing (FNP/APRN) or Physician Assistant Studies
• Minimum of 1-2 years of clinical experience in Family or Internal Medicine preferred
• Experience caring for patients of all ages and managing acute and chronic conditions strongly preferred
Licensure, Certification & Registration
• Active Florida APRN or PA-C license
• Active DEA license
• ANCC, AANP, or NCCPA (or equivalent) certification
• BLS required; ACLS (preferred)
