Job description

The Division of Cardiology at Albany Medical College is seeking a full-time Nurse Practitioner to join our busy and collaborative outpatient cardiology practice. This position offers the opportunity to provide high-quality cardiovascular care in a supportive, team-oriented environment while working closely with cardiologists and multidisciplinary care teams.

This role will primarily focus on outpatient follow-up visits and pre-operative cardiovascular evaluations, supporting continuity of care for patients with a wide range of cardiac conditions.

Position Highlights

  • Full-time Monday-Friday 8am-4:30pm

  • Outpatient clinic-based role

  • Average 15 patients per day once fully trained with 2 half days of admin time (8 patients on admin days)

  • Collaborative practice with cardiologists, nurses, and ancillary support staff

  • Opportunity to build expertise in outpatient cardiovascular management

Responsibilities

  • Conduct outpatient follow-up visits for patients with cardiovascular conditions such as coronary artery disease, heart failure, arrhythmias, hypertension, and valvular heart disease

  • Perform pre-operative cardiovascular evaluations and risk assessments for patients undergoing surgical procedures

  • Obtain detailed medical histories and perform focused cardiovascular examinations

  • Order and interpret diagnostic studies including laboratory tests, ECGs, and cardiac imaging as appropriate

  • Develop and implement treatment plans in collaboration with supervising cardiologists

  • Provide medication management and patient education on cardiovascular disease prevention and management

  • Coordinate care with primary care providers, surgeons, and other specialists

  • Document patient encounters and treatment plans accurately in the electronic health record

  • Participate in quality improvement initiatives and practice efficiency efforts

Qualifications

  • Current license and active registration as a Nurse Practitioner or Physician Assistant in New York State

  • Master's degree from an accredited Nurse Practitioner program or completion of an accredited Physician Assistant program

  • Interest in or experience with cardiovascular medicine preferred

  • Familiarity with electronic medical records; Epic experience preferred

  • Strong communication and collaboration skills in a multidisciplinary clinical environment

  • Commitment to patient-centered care and maintaining HIPAA confidentiality standards
