Clinical Liaison - Rehabilitation & Skilled Nursing

Baton Rouge General is seeking a Clinical Liaison to coordinate admissions for the Inpatient Rehabilitation and Skilled Nursing units. This position evaluates referrals, determines admission eligibility, obtains insurance authorization, and serves as a resource to patients, families, physicians, and referral sources.

Responsibilities
  • Assess referred patients for admission appropriateness and eligibility.
  • Review medical records and conduct patient evaluations.
  • Coordinate insurance verification and authorization processes.
  • Educate patients, families, and referral sources regarding available services.
  • Develop pre-admission plans of care.
  • Maintain referral tracking and admission statistics.
  • Build and maintain relationships with physicians, case managers, and community partners.
  • Participate in outreach, marketing, and business development activities.
Qualifications
  • Minimum of two (2) years of clinical experience in inpatient rehabilitation, care coordination, case management, or related healthcare setting.
  • Clinical licensure or certification required; Louisiana RN license preferred.
  • Knowledge of rehabilitation services, skilled nursing admissions, and payer guidelines.
  • Strong communication, organizational, and customer service skills.
