What You'll Do:
  • Welcome new residents and families with professionalism and compassion.
  • Complete admission assessments, documentation, and care planning.
  • Coordinate with interdisciplinary team members to ensure smooth transactions.
  • Provide resident and family education about care, routines, and expectations.
  • Ensure regulatory compliance and accuracy in admission documentation.
  • Support the nursing team as needed during the shift.

  • Current RN license in Illinois (required)
  • Long-term care experience preferred, but not required - strong mentorship provided.
