Job description

The RN Case Manager delivers integrated nursing and case management services for individuals with behavioral health, substance use, and medical needs. This role provides assessments, direct nursing care, care coordination, and health education while collaborating with interdisciplinary teams and community providers to support continuity of care, recovery, and improved health outcomes. 

POSITION SPECIFIC DUTIES & RESPONSIBILITIES:    

  • Provide professional nursing care to individuals with serious mental illness, substance use disorders, and cooccurring conditions. 
  • Conduct and document nursing assessments. 
  • Administer and monitor prescribed medications in accordance with provider orders. 
  • Support health promotion and disease management by providing nursing education related to behavioral health conditions, physical health comorbidities, medications, relapse prevention, and wellness activities within the RN scope of practice. 
  • Coordinate nursing care and services with treatment team to promote continuity of care and safe treatment transitions. 
  • Provide nursing follow-up after hospital or emergency department utilization. 
  • Monitor client engagement, service utilization, and adherence to treatment plans and clinical guidelines, identifying changes in condition or risk and escalating per protocol. 
  • Apply evidence based and trauma informed nursing principles in the care of individuals. 
  • Provide nursing crisis intervention. 
  • Engage clients and, with appropriate consent, families or designated supports in nursing education, care coordination, and reinforcement of provider directed treatment plans. 
  • Accompany clients to medical or behavioral health appointments as needed to provide nursing support, education, and advocacy within the RN role. 
  • Assist clients in accessing community based resources to support stabilization, recovery, and whole person wellness. 
  • Maintain documentation of nursing assessments, interventions, medication administration, client responses, and coordination activities in compliance with organizational policy, OBN standards, and applicable regulations. 
  • Participate in quality improvement, utilization review, and outcome monitoring activities, to support program effectiveness and compliance. 

QUALIFICATIONS 

education: 

  • Bachelor's Degree in Nursing (BSN) Preferred 
  • Associate's Degree in Nursing (ADN) Required. 

EXPERIENCE: 

  • ADNprepared nurses must have a minimum of thirty-six (36) months of professional nursing experience in behavioral health and/or substance use disorder treatment. 
  • BSNprepared nurses preferred to have a minimum of six (6) months of professional nursing experience in behavioral health and/or substance use disorder treatment

CERTIFICATIONS/LICENSES: 

  • Must be a licensed Registered Nurse in the State of Oklahoma 
  • ODMHSAS Case Manager II Certification, completed within 60 days of eligibility. 
  • Must possess a valid Driver License and satisfactory driving record to use agency and/or personal automobile to travel to locations other than primary office and/or for the transportation of clients. 

SAFETY SENSITIVE JOB CLASSIFICATION: 

This job is classified as a “safety-sensitive” position as defined by the Oklahoma Medical Marijuana and Patient Protection Act. Due to the “safety-sensitive” classification, an employee in this position would be subject to drug and alcohol testing, including random testing. Marijuana is one of the substances included in the drug panel screening. Possession of a medical marijuana license will not excuse you from the testing process or the consequences of testing positive for marijuana per the Family & Children’s Services Drug Free Workplace Policy, including possible revocation of a job offer or dismissal from employment.
