ABOUT THE MEDICAL DIRECTOR ROLE

As we continue to scale Upperline Plus in our regions, Upperline Health is seeking a Medical Director for the Augusta, GA market to advance clinical excellence, strengthen provider engagement and lead clinical value-based care across the market.

This role combines direct patient care, clinical leadership, and VBC program management and partners closely with the central interdisciplinary team, population health and operations teams.

With a patient-centered approach, the Medical Director will see a small panel of their own patients, provide clinical oversight and hands-on coaching to Upperline Plus' nurse practitioners, identify market opportunities, improve clinical processes, and lead VBC and population health initiatives - all with a focus on optimizing the care model to improve patients' outcomes and quality of life.

Must reside in the Augusta, GA area.

Approximately 30-50% of time will be spent in the clinic setting.

Required Experience and Skills
  • Doctor of Medicine (M.D.) or Doctor of Osteopathic Medicine (D.O.)
  • Board certified in Internal Medicine, Family Medicine, Preventative Medicine, or Geriatrics
  • Active license in Georgia and willing to pursue multi-state licensure if needed
  • ~ 5 years of outpatient practice experience, inclusive of caring for adult / senior, high-risk, polychronic, clinically complex patient populations
  • Clinically practiced in a value-based care model and/or championed programs to improve quality, patient outcomes and reduce risk
  • Previous clinical oversight or coaching of other providers
  • Must reside in market; be willing and able to provide direct care to a small panel of patients and routinely visit clinics and providers within the market
Preferred Experience and Skills
  • Exposure or experience improving or implementing clinical processes, protocols, or evidence-based guidelines
  • Experience or expertise in VBC programs (e.g., Transitional Care Management, Complex Care Management, ED Diversion, Advanced Care Planning, High Risk Patient Outreach)
