Grundy Center Family Medicine is seeking a compassionate and patient-focused LPN to join a collaborative clinic team dedicated to providing high-quality care to patients and families in the community. This role offers the opportunity to build meaningful relationships with patients while working closely with providers and clinical staff in a supportive outpatient setting.

Responsibilities

Prepare patients for appointments, exams, treatments, and minor procedures

Obtain vital signs, patient history, and other clinical information

Administer medications, vaccines, and other treatments within scope of practice

Assist with specimen collection and preparation for laboratory testing

Support patient follow-up care, referrals, and coordination of services

Document patient information accurately within the electronic health record
-
Provide education and guidance to patients and families during clinic visits
-
Collaborate with providers and clinic staff to support safe, efficient patient care
-
Maintain compliance with safety, privacy, and regulatory standards
-
Contribute to a positive, team-focused clinic environment centered on excellent patient care
Qualifications
Education:
License(s)/Certification(s):
- Current license to practice nursing in the State of Iowa/Illinois/Wisconsin for Licensed Practical Nurse.
