LPN - Brookside Memory Care
Cross Keys Village - The Brethren Home Community (CKV) is currently seeking a Licensed Practical Nurses to become an integral leader on our Brookside team and help provide our residents with the highest quality of care. As an LPN Coordinator working at CKV, we will provide you with opportunities to enjoy a friendly work environment guided by Christian values.
Our current needs include;
  • Part-time Night Shift; working 10:30 p.m. - 6:30 a.m. 37.5 hours every two weeks. Weekend and holiday rotation required.

Our Brookside Memory Care Residence is a specialized personal care environment for individuals with a diagnosis of neurocognitive disorder or dementia, designed to provide an optimal environment during the intermediate stages of their disease. CKV team members work together to provide Brookside residents with guidance, cueing, the right level of stimulation and interaction with others who understand the progression of their condition.
Job Description:
As an LPN, your scope of responsibilities will include but is not limited to:
  • Overseeing and performing routine nursing duties within the scope of LPN practice.
  • Completing multiple nursing rounds per shift to evaluate status of the residents, new admissions and/or transfers. Evaluating nursing care and suggesting modifications as necessary.
  • Evaluating resident changes in condition. Notifying physician to obtain new orders or to send for emergent care. Coordinating specialty services i.e. wound care, psychiatric services, hospice, lab services, radiology, hearing and vision.
  • Responding to emergencies at Brookside and utilizing critical judgment to ensure timely and appropriate care/documentation is provided.
  • Identifying areas of opportunity for QAPI and communicating to the personal care administrator. Completing audits as assigned.
  • Obtaining prior-authorizations for medications; management of controlled substances and coordinating refills or substitution of medications.
  • Identifying team, resident or family concerns.
  • Providing oversight and leadership to the personal care attendants and resident assistants.
  • Seeking to resolve challenging situations by soliciting feedback from the RN or administrator.
  • Leading the resident admission process to ensure timely and thorough completion.
  • Participating in resident care meetings.
  • Reviewing daily 24-hour report and collaborating with team members to ensure resident medical needs are met.
  • Completing required documentation and supplements direct care documentation as necessary.
  • Educating personal care attendants and resident assistants on wound care and medication administration. Providing remedial training to medication technicians.
  • Guiding the direct care team in planning and organizing work on the shift.
  • Point of contact for personal care team members. Accepting call offs and adjusting the schedule accordingly to ensure adequate coverage for personal care.
  • Educating the direct care team on aging in place; identifying equipment or services needed to promote aging in place.
  • May assist with performing medication administration, treatments or ADL care.
  • Leading the direct care team in post-mortem care. Notifying family of resident's death and communicating with physician and funeral home.
  • Directing and providing performance evaluations with identified direct care team member(s).
Education and Experience:
  • Must be licensed as a practical nurse in the State of Pennsylvania.
  • Post-licensure experience preferred but not required.
  • Must demonstrate CPR competencies.
  • Evidence of continuing education in line with community, state and federal regulations.
  • Must possess quality mindedness, critical thinking, initiative in leadership responsibilities, interpersonal sensitivity and communication skills.
