Alta Mira Specialized Family Services Job Post

Alta Mira is a non-profit organization that has been supporting individuals with developmental or intellectual delays or disabilities, and their families, since 1985. We provide services to all age groups. We support over 800 children annually through our Early Intervention Program and 1500 adults in our Family Support Services. We employ over 60 employees and 250 independent contractors. We are an excellent employer with a high level of schedule flexibility and very rich benefits. Experience: BA/BS preferred, or 2 years of related Developmental Disability (DD) experience.

Position Specific Essential Job Functions
  1. Performs essential job functions with or without reasonable accommodation at an acceptable level.
  2. Initiate and maintain family and individual relationships to ensure Alta Mira Family Support Services (FSS) are delivered.
  3. Partner with families, as the experts on care for their family members with developmental disabilities and ensure family participation in the training of direct care providers.
  4. Gather and communicate family input regarding service quality.
  5. Implement and monitor Family Support Services for those receiving services.
  6. Develop nurturing relationships with clients to facilitate desired outcomes and meet needs.
  7. Take initial referrals, schedule intake interviews, and complete them to gather information for planning service delivery.
  8. Develop individualized goals and objectives (Individualized Family Service Plan / IFSP or Individual Service Plan / ISP) in collaboration with clients, consulting with other Family Support Services staff and/or agencies as appropriate.
  9. Schedule and conduct regular home visits and/or observation visits to review goals, service quality, health, and safety needs.
  10. Respond to requests from families and/or individuals for changes in program or service plans.
  11. Provide ongoing Family Support Service coordination for clients.
  12. Follow Service Coordination policies and procedures as outlined by funding and regulatory agencies, including Alta Mira Specialized Family Services.
  13. Serve as a resource for clients and other community providers, sharing policy and procedural information.
  14. Assist clients in locating and accessing needed services within Alta Mira or through other providers.
  15. Deliver and monitor services to ensure they are provided in natural environments and integrated into the individual's daily routine.
  16. Collaborate with families, program participants, and community resources to promote independence.
  17. Develop, plan, lead, and participate in educational, training, and outreach activities, including planning and delivering lectures or workshops as requested. Share pertinent information with supervisors, team members, and the agency's services and/or programs.

Participate in the development and implementation of Family Support Services training and orientation programs.

  1. Collaborate with the Family Support Services Program Director, staff, and families or individuals receiving services to develop a comprehensive training and orientation program for all service components.
  2. Conduct an initial introduction and basic orientation program for direct care providers.
  3. Schedule and conduct individual orientations and home visits or studies with prospective independent contractors.
  4. Perform face-to-face home visits monthly.
  5. Communicate with direct care providers and assist them in understanding the specific needs of people receiving services.
  6. Participate in the overall development and implementation of the FSS program.
  7. Complete and submit accurate documentation and reports as required, adhering to program standards and deadlines.
  8. Ensure all family and individual files are complete, current, and compliant with funding and regulatory standards.
  9. Complete document notes for each contact made with families, individuals, and/or direct care providers.
  10. Review and approve provider notes, facilitating corrections as needed.
  11. Maintain and manage client contacts as necessary.
  12. Ensure required data is collected, compiled, and reported.
  13. Comply with accreditation and regulatory standards as required.
  14. Crisis/Emergency Intervention.
  15. Be available for on-call duties as assigned.
  16. Accompany staff and/or people receiving services to the emergency rooms when appropriate.
  17. Make referrals to community resources when individuals receiving services are in crisis.
  18. Perform other duties as required.
