In this role, you will work within the client's residence, providing a personalized healthcare experience. Key Responsibilities
  • Deliver direct physical therapy interventions aimed at preventing disabilities, alleviating pain, and optimizing muscle function as outlined in individualized treatment plans prescribed by physicians.
  • Supervise and mentor Physical Therapy Assistants, ensuring precise implementation of care plans and adherence to best practice standards.
  • Conduct thorough evaluations of clients’ functional levels through diagnostic tests that assess muscle, nerve, joint function, and overall capabilities, leading to comprehensive treatment plans.
  • Assess home environments to identify modifications that enhance client independence during rehabilitation.
  • Administer therapeutic exercises, manual therapy techniques such as massage, and utilize advanced physical agents to promote client recovery.
  • Fit and adjust prosthetic and orthotic devices, collaborating effectively with healthcare providers to recommend necessary modifications.
  • Maintain accurate clinical records, including initial evaluations, care plans, progress notes, and discharge summaries, ensuring compliance with regulatory standards including OASIS documentation.
  • Actively participate in interdisciplinary team case conferences, sharing insights and contributing to client treatment evaluations and adjustments.
  • Educate clients and their families about therapeutic exercises and the proper use of physical therapy devices, fostering independence and promoting proactive health management.
Patient Population / Client Focus

This role focuses predominantly on clients with orthopedic needs, ranging from post-operative recovery to injury rehabilitation. Qualifications
  • A Doctor of Physical Therapy (DPT) degree from an accredited institution.

  • Current, valid PT licensure in the state of New York.
  • Valid CPR certification and a clean driver’s license with full insurance coverage.
