Pinnacle Flex Wellness Clinic is seeking a dedicated and compassionate Orthopedic Physical Therapist to join our team in Niagara Falls, New York. This full-time, onsite role will play a crucial part in enhancing the quality of life for our clients by delivering expert orthopedic care and rehabilitation services in the comfort of their own homes. In this role, you will work within the client's residence, providing a personalized healthcare experience. Each day will present you with unique challenges as you assess and adapt therapy interventions based on the specific needs of each client. You will collaborate closely with other healthcare professionals in an interdisciplinary team to ensure holistic care and optimal therapy outcomes.

• Deliver direct physical therapy interventions aimed at preventing disabilities, alleviating pain, and optimizing muscle function as outlined in individualized treatment plans prescribed by physicians.
• Supervise and mentor Physical Therapy Assistants, ensuring precise implementation of care plans and adherence to best practice standards.
• Conduct thorough evaluations of clients’ functional levels through diagnostic tests that assess muscle, nerve, joint function, and overall capabilities, leading to comprehensive treatment plans.
• Assess home environments to identify modifications that enhance client independence during rehabilitation.
• Administer therapeutic exercises, manual therapy techniques such as massage, and utilize advanced physical agents to promote client recovery.
• Fit and adjust prosthetic and orthotic devices, collaborating effectively with healthcare providers to recommend necessary modifications.
• Maintain accurate clinical records, including initial evaluations, care plans, progress notes, and discharge summaries, ensuring compliance with regulatory standards including OASIS documentation.
• Actively participate in interdisciplinary team case conferences, sharing insights and contributing to client treatment evaluations and adjustments.
• Educate clients and their families about therapeutic exercises and the proper use of physical therapy devices, fostering independence and promoting proactive health management.

This role focuses predominantly on clients with orthopedic needs, ranging from post-operative recovery to injury rehabilitation. Our patients, often elderly or dealing with chronic conditions, require targeted interventions to regain mobility and improve their quality of life. The personalized nature of our service allows for strong relationships to be built between therapists, clients, and families, which is crucial for successful outcomes.

• A Doctor of Physical Therapy (DPT) degree from an accredited institution.
• A minimum of one year of physical therapy experience, preferably in acute care or home care settings.
• Current, valid PT licensure in the state of New York.
• Valid CPR certification and a clean driver’s license with full insurance coverage.
• Exceptional clinical skills with a strong commitment to patient-centered care, integrity, and excellence.
• Strong problem-solving abilities and decision-making skills, with the capacity to work independently as well as part of a collaborative team.
• Excellent communication and interpersonal skills to effectively engage with clients and family members.
• Familiarity with federal and state regulations governing physical therapy practices and home healthcare services.

• Prior experience in home health care settings is highly desirable, particularly with orthopedic patients.
• Proven ability to mentor and lead a team of Physical Therapy Assistants.
• Experience in interdisciplinary collaboration and case management.
