What You'll Do

As a Physical Therapist Assistant, you will work under the direction of a licensed Physical Therapist to deliver exceptional home health services and support positive patient outcomes.

Key Responsibilities
  • Provide physical therapy treatments and patient education in the home according to the physician-approved Plan of Care.
  • Complete patient visits and documentation accurately, timely, and in compliance with agency standards.
  • Monitor patient progress and promptly communicate any changes in condition, treatment response, or scheduling needs to the supervising therapist or nurse.
  • Collaborate with physicians, nurses, therapists, and other members of the interdisciplinary care team to coordinate patient care.
  • Participate in care conferences and contribute to patient care planning as appropriate.
  • Adhere to all safety protocols, infection prevention procedures, and proper equipment-use guidelines.
  • Protect patient privacy and maintain strict confidentiality in accordance with HIPAA and agency policies.
  • Ensure compliance with agency policies and all applicable state, federal, and professional regulations.
  • Participate in ongoing education, staff meetings, and quality improvement initiatives.
  • Perform other duties as assigned to support patient care and organizational goals.
What You'll BringRequired Qualifications
  • Current and unrestricted Physical Therapist Assistant license in the state of Indiana.
  • Graduate of an accredited Physical Therapist Assistant program approved by the American Physical Therapy Association (APTA).
  • Minimum of six (6) months of experience as a Physical Therapist Assistant.
  • Strong verbal, written, and interpersonal communication skills.
  • Reliable transportation with a valid driver's license, current automobile insurance, and a satisfactory driving record.
