Job description

Overview

The Registered Nurse (RN) functions under the general supervision of the Nursing Support Specialist (NSS) while exercising independent professional judgment to deliver high-quality, person-centered care to individuals with intellectual and developmental disabilities. The RN upholds and promotes the mission, vision, and values of Heritage Christian Services, Inc.

Pay rate for this position:  $70000 / yr - $76500 / year

The salary of the finalist selected for this role will be set based on a variety of factors, including but not limited to departmental budgets, qualifications, experience, education, licenses, specialty, and training. The above range represents the organization's good faith and reasonable estimate of the range of possible compensation at the time of posting

Responsibilities

Clinical Care & Health Oversight (55%)

  • Conducts ongoing health assessments and supports individual's health needs in the certified residential, day habilitation and respite settings.
  • Provides clinical response and guidance for health concerns and medical emergencies.
  • Coordinates medical care including collaboration with physicians, specialists, pharmacies, and medical liaisons. Additionally, participates as a member of the interdisciplinary team at HCS to support person-centered care planning.
  • Works in collaboration with the Medical Liaison to facilitate timely scheduling, appointment attendance, and follow-through on medical recommendations resulting from appointments/hospitalizations.
  • Develops, implements, and maintains individualized Plans of Nursing Service to outline nursing care and define delegated responsibilities for Direct Support Professionals (DSPs).
  • Oversees medication management, including MAR review, reconciliation, and follow-up on medication errors.

Delegation, Training & Staff Education (25%)

  • Delegates nursing tasks to DSPs in accordance with the NYS Nurse Practice Act.
  • Trains, supervises and evaluates DSP competency in health-related supports and delegated nursing tasks.

Regulatory Compliance & Quality Improvement (20%)

  • Supports agency survey readiness efforts, including participation in on-site COMPASS surveys and remediation of identified deficiencies.
  • Participates in incident management, including clinical review of reportable incidents
  • Ensures compliance with OPWDD regulations, agency policies, and administrative memoranda

Qualifications
  • New York State Department of Education licensure as a Registered Nurse
  • Experience with person(s) with developmental disabilities and long-term care experience are preferable. Ability and temperament to work effectively with people with intellectual and physical disabilities is required
  • Valid driver's license in compliance with Heritage Christian Services' motor vehicle record standards, and reliable transportation to and from work
