Job Details

This role is in support of Cencora's patient and provider support and pharma commercialization services in Canada marketed through our Innomar Strategies business.

Responsibilities:

  • Provide support to patients and health care providers by offering information on the Patient Support services, specific diseases, and therapies based on manufacturer provided materials and product monograph;

  • Assist patients and caregivers in conducting assessments related to Patient Support services;

  • Act as a liaison between patients, families and health care providers related to medication access.

  • Address general inquiries regarding program services, disease states, or product related queries based on provided materials;

  • Offer continuous support on disease states or product-related questions to ensure patient adherence;

  • Refer the patient to their healthcare providers for clinical questions beyond program materials;

  • Document patient information and interactions within the electronic patient record;

  • Conduct ongoing file review and updates to reflect patient journey;

  • Follow case-management protocols, including data collection, patient follow-up, therapy disruption/discontinuation monitoring, adverse event reporting and liaison with primary healthcare providers;

  • Collaborate with the reimbursement specialist and the physician to secure funding for their patient's therapies and explore alternative funding options;

  • Ensure patient services are coordinated and tracked, according to KPI's, resulting in expedited access to therapy;

  • Participate in specialized training sessions for specific therapies provided by the pharmaceutical team;

  • Attend pharmaceutical meetings, patient meetings, conferences, and work with patient associations and advocacy groups as needed ;

  • Coordinate with health care providers, pharmacies and infusion/injection clinics for treatment logistics and issue resolution;

  • Maintain clear communication with the manager, team and the client regarding the management of patient cases;

  • Assist in the training process of new Nurse Case Managers and provide feedback on protocols, databases, processes and inter-departmental processes;

  • Report Adverse Events / Severe Adverse Events following program procedures;

  • The Nurse Case Manager will also be assigned other duties and tasks as required from time to time. May be required to be on standby/ on call as part of this role.

Education:

  • A Diploma in health care or a related field, with a preference for Bachelor's degree

  • Licensed Nurse (RN or RPN/LPN) with active registration from the College of Nurses in the province of employment is required.

Work Experience:

  • Minimum of two (2) to five (5) years in the healthcare, pharmaceutical, insurance or related fields;

  • Ideal candidates should be nurses with healthcare and case management background in medical/hospital settings, insurance or bio-pharmaceutical industry;

  • Fluency in French and/or English is required

PRINCIPALES FONCTIONS ET RESPONSABILITES :

Fournir un soutien medical aux patients et aux fournisseurs de soins de sante, y compris, mais sans s'y limiter, l'education sur des maladies et traitements specifiques et la gestion des maladies;

Fournir un soutien telephonique (incluant les appels de maintenance et d'appui a l'observance entrants et sortants) selon les besoins;

Gestion de la base de donnees et verification continue des dossiers et de l'information saisie;

Administrer et adherer a des protocoles de gestion des cas, incluant la surveillance clinique, la collecte de donnees, le suivi continu des patients, la conformite, le suivi de l'interruption ou l'abandon de traitement, les rapports sur les effets indesirables et la liaison avec les fournisseurs de soins de sante primaires;

Se preparer et assister a des seances de formation specialisee plus poussees sur des therapies specifiques, donnees par l'equipe pharmaceutique;

Travailler directement avec le specialiste du remboursement et le medecin pour obtenir des fonds pour les traitements de leurs patients; aider a trouver d'autres sources de financement au besoin;

Assister aux reunions de ventes pharmaceutiques au besoin;

Assister aux reunions de patients et aux conferences au besoin pour eduquer les patients et les prestataires concernant les informations sur les maladies/les produits et les services de gestion de cas;

Travailler avec les associations de patients et les groupes de defense;

Etablir et maintenir des partenariats avec l'equipe pharmaceutique;

Assurer la liaison avec les fournisseurs de soins de sante, les pharmacies et les cliniques de perfusion et d'autres organismes au besoin, afin de coordonner les perfusions, l'administration de medicaments et resoudre les problemes qui surviennent;

Entretenir des communications claires avec le gestionnaire, a l'equipe et le client concernant la gestion des cas de patients;

Avec les gestionnaires, participer au processus de formation de nouvelles infirmieres gestionnaires de cas, y compris, mais sans s'y limiter, l'observation et la surveillance des appels;

Soutenir et collaborer avec le personnel d'autres programmes en vue de l'elaboration et de la mise en uvre de programmes de gestion des maladies, incluant l'etablissement des exigences cliniques, la redaction de textes, de manuels ou de depliants pour les patients, la determination des parametres du programme de conformite, des evaluations des besoins et des protocoles;

Fournir des commentaires au sujet des protocoles, bases de donnees, processus et relations entre departements actuels

Etre disponible sur appel en dehors des heures regulieres et le week-end (24/7) pour des consultations telephoniques periodiques et des courriels en fonction des besoins du programme;

Se charger de signaler les effets indesirables aux unites/departements qui lui sont confiees;

D'autres fonctions et taches peuvent etre confiees a l'infirmiere gestionnaire de cas de temps a autre, selon les besoins;

Peut etre appele a travailler sur appel ou de garde.

EXPERIENCE ET NIVEAU D'EDUCATION REQUIS:

Baccalaureat ou diplome d'infirmier(ere) autorise(e) (IA), diplome d'etudes superieures ou experience specifique de gestion d'un cabinet est preferable, avec cinq (5) annees d'experience en soins infirmiers;

Au moins deux (2) ans d'experience du contexte du remboursement par des tiers publics ou prives ou dans les ventes (non requis, mais un atout), la gestion des soins ou le soutien clinique dans l'industrie pharmaceutique, ou experience connexe;

Le(la) candidat(e) prefere(e) est un(e) infirmier(ere) avec une experience dans les soins de sante et la gestion de cas, dans un cabinet medical ou un hopital ou dans l'industrie de l'assurance ou de la biopharmaceutique;

Bilinguisme (francais et anglais) est preferable;

Experience en hematologie et/ou nephrologie constitue un atout.
