Case Manager - Substance Abuse

Assignment Details
Contract Length: 13 weeks
Guaranteed Hours: 72 hours biweekly
Requirements
- Bachelor's Degree (BA) or equivalent; or Associate's degree in Human Services field
- 2-3 years related experience or training in the area of chemical dependency
- Certified Addiction Counselor (CAC) preferred
- Ability to complete on-site CPI and CPR training/certification
Roles & Responsibilities
- Maintain a caseload that includes conducting individual intake evaluations and treatment planning
- Complete documentation in compliance with regulations in a timely manner
- Coordinate with collaterals and referral sources as needed
- Assist in the retrieval of patient medications from local pharmacies
- Support staff with milieu management and crisis management
- Coordinate transportation needs for appointments and aftercare
- Engage in identifying and locating contraband and performing unit searches as necessary
- Assess patients' continuing care needs and arrange appropriate programming
- Educate and lead group programming sessions
