We are seeking a(an) Clinical Rehab Specialist PRN for our team to ensure that we continue to provide all patients with high quality, efficient care. Did you get into our industry for these reasons? Position Summary: 


The Clinical Rehabilitation Specialist contributes to high quality, patient-centered care by identifying candidates for inpatient rehabilitation and initializing timely, appropriate patient transfers. In collaboration with the Rehab Medical Director, the Clinical Rehabilitation Specialist ensures appropriate patient placement through clinical assessments and screening. The Clinical Rehabilitation Specialist serves as an inpatient rehabilitation expert between internal and external referral sources and inpatient rehabilitation operations. The Clinical Rehabilitation Specialist is responsible for establishing relationships with case managers, physicians, payors, and other referral sources. 


Major Responsibilities: 


  • Assists and coordinates intake and pre-admission screening processes. Documents evidence of rehab admission criteria within established timeframe. 
  • Performs on-site and off-site clinical evaluations to assist Rehab Medical Director in determining patient eligibility for admission. 
  • Provides information to patient/family/caregiver related to inpatient rehab and program features. Communicates expectations and requirements for participation in the rehab program, and conducts or coordinates tours of the rehab unit. 
  • Obtains information relating to patient resources and healthcare insurance benefits. 
  • Demonstrates clinical knowledge of rehab admission criteria, leading to appropriate identification of qualified rehab patients. Advises referral sources on inpatient rehab criteria. 
  • Establishes and maintains effective working relationships with case management and referring physicians, and serves as the primary point of contact. 
  • Ensures timely follow-up communications to referral sources regarding their patients and hospital programs. 
  • Facilitates educational contacts with physicians, case managers, interdisciplinary members of the care team, and other professionals. Serves as an educational resource to patients, family members and caregivers. 
  • Participates in market analysis, data analysis, and development and implementation of business plans for the Rehab Unit. Contributes to the development and maintenance of referral source information, such as brochures, flyers, and other outreach tools. 
  • Assists in developing the annual strategic plan for the Rehab Unit/Hospital, including visible community outreach programming to raise community awareness of available services. 
  • Develops and implements a system to ensure 24-hour call response. 
  • Performs other duties as assigned. 
  • Practices and adheres to the “Code of Conduct” and “Mission and Value Statement.”   

 Qualifications: 


  • Currently licensed as a registered professional nurse, or other licensed professional in specialty, in the state(s) of practice and/or has an active compact license, in accordance with law and regulation. 
  • Certified Registered Rehab Nurse (CRRN) is preferred 
  • Basic Life Support (BLS) certification 
  • Associate Degree in Nursing or Respiratory Therapy or Bachelor’s Degree in Physical or Occupational Therapy 
  • Master’s Degree in Speech Therapy 
  • 2+ years experience in clinical care 
  • 2+ years experience in inpatient rehabilitation 
  • Experience as a clinical evaluator
