Job description

Schedule: M-F (9:00am - 12:30pm), every other weekend Sa-Su (9am - 12:30pm) - there is an option to pick up more clients if you want more hours

Primary Responsibilities for PCW Milwaukee Jobs Under the supervision of an RN Case Manager, you will provide hands-on care and assistance with daily living activities based on a written Care Plan.

  • Provide hands-on personal care, including assistance with bathing, personal hygiene, grooming, toileting, and dressing.
  • Assist clients with basic mobility, ambulation, and safe, standard transfers.
  • Plan and prepare nutritious meals and snacks according to the client’s dietary needs.
  • Perform light housekeeping tasks to maintain a clean, safe, and healthy home environment.
  • Report all observations and any changes in a client’s condition to the RN Case Manager.
  • Accurately document all care and observations from your visit using our Electronic Visit Verification (EVV) system.


Requirements for Home Health Care Jobs Milwaukee

  • Minimum 1 year of experience as a Personal Care Worker (PCW) or similar role is required.
  • Able to speak Spanish. The client you will be assisting speaks Spanish.
  • Previous experience in a home health care setting.
  • A valid driver’s license and a reliable, insured vehicle to travel between client homes in Metro Milwaukee.
  • Must have a positive attitude, be a team player, and be open to taking direction.
  • Must be reliable, on-time, and able to keep a consistent schedule.
  • Able to pass a pre-employment physical and drug screen.


Preferred Qualifications

  • Experience caring for seniors or clients with conditions such as dementia.
  • A solid foundation in basic PCW skills and a willingness to learn new care techniques.
