Lead Care Professional
CARE IN HOME, LLC dba Home Instead
Position Overview
The Lead Care Pro position will perform all duties under the Care Pro job description along with additional duties to support key player job functions which could include the following: Care Pro engagement activities, Care Pro training, Care Pro & client inquiries, and client care support.
Essential Job Functions
  • Performs all duties under the Care Pro job description as needed.
  • Lead Care Pro engagement activities which could include:

o Cover open shifts
o Meet Care Pro for SOCs
o Increase meaningful participation and connections at Care Pro meetings/events
o Participate in planning & executing Care Pro quarterly meetings
o Care Pro Focus Group participation and support
o Be a resource/mentor for other Care Pros
  • Lead Care Pro training activities which could include:

o Help conduct new Care Pro orientation, on site, as needed
o Encourage and remind Care Pros to attend training classes
o Provide personal skills training for Care Pros in clients' homes
o Perform shadow opportunities to new Care Pros
o Offer feedback to key player regarding new Care Pros progress/needs
o Perform Intros for new Care Pros, as needed using the Client/Care Pro Introduction checklist
  • Client care support which could include:

o Contact new clients to ensure quality care as directed by the client care team.
o Communicate changes in condition to client care team.
o Engaged in Start of Care (SOC) with Client care team
  • Other duties as assigned.
