Job Profile Summary

This role focuses on providing professional and nonprofessional nursing care services in accordance with physician orders. In addition, this role focuses on performing the following Home Care Nurse duties: Assesses, plans, implements, and evaluates the care needs of a designated group of patients or provides nursing care and identified clinical support tasks within the scope of practice for achievement of the patient's plan of care as directed. Provides professional nursing care to patients. Requires a nursing license. A professional individual contributor role that may direct the work of other lower level professionals or manage processes and programs. The majority of time is spent overseeing the design, implementation or delivery of processes, programs and policies using specialized knowledge and skills typically acquired through advanced education. An experienced level role that applies practical knowledge of job area typically obtained through advanced education and work experience. Works independently with general supervision, problems faced are difficult but typically not complex, and may influence others within the job area through explanation of facts, policies and practices.

Job Overview

The Hospice RN provides delivery of quality hospice nursing care for patients in a variety of settings, including patients under home health and hospice plans of care, residing in private homes, assisted living, skilled nursing facilities and hospice inpatient facilities. Hospice RN care for patient and families that is reflective of the organization's core values. The Hospice RN is knowledgeable about and supportive of the primary nursing model.

Job Description

Minimum Qualifications:

1. Massachusetts RN Licensure.

Location: High Pointe House, Haverhill, MA

Hours: Per Diem

Duties and Responsibilities: The duties and responsibilities listed below are intended to describe the general nature of work and are not intended to be an all-inclusive list. Other duties and responsibilities may be assigned.

1. Provides hospice nursing care to patients and families utilizing current principles of palliative care and symptom management to produce hospice outcomes.

2. Coordinates care in a cost-efficient manner.

3. Collaborates effectively with other members of the Interdisciplinary Team.

4. Utilizes nursing process and current standards of nursing practice in providing hospice care.

5. Remains competent on technical nursing skills (i.e. IVs, Gtubes, etc.)

6. Provides effective patient and family teaching.

7. Communicates with Medical Director, clinical, and support staff as indicated regarding changes in patient's condition.

8. Understands family dynamics and works effectively within various types of family systems.

9. Provides peers with support and mentoring if appropriate.

10. Utilizes resources appropriately, demonstrates the ability to act as a patient and family advocate and provides information to families about all available resources.

11. Accurately addresses need for additional services. Communicates patient reports and status changes concisely and effectively.

12. Assesses patient for need for Fall Precautions. Updates Care Plan, Medication Profile and HHA Treatment Plan routinely and as patient condition changes.

13. Completes all nursing documentation per procedures and nursing standard of practice including assessments, interventions, responses to interventions, communications, verbal orders, etc.

14. Manages assignment efficiently and effectively.

15. Involves Team Leader and Medical Director in situations appropriately.

16. Understands how to maintain professional boundaries with patients and families.

17. Utilizes good customer service skills internally and externally.

18. Assists in orientation of new staff and mentors LPNs and nursing students as requested.

19. Pursues standards of excellence in performance as a Hospice RN.

20. Works with Team Leader and Clinical Manager to identify opportunities for improvement in performance and works to address them.

21. Proactive in increasing knowledge of hospice care and standards of practice.

22. Participates in achieving goals and objectives.

23. Represents promotes hospice care in the community and other healthcare professionals.

Physical Requirements:

1. Prolonged, extensive, or considerable standing/walking, and climbing stairs.

2. Lifts, positions, pushes and/or transfer patients and equipment up to 100 pounds.

3. Considerable reaching, stooping, bending, kneeling, crouching, or crawling.

4. Ability to use hands to finger, handle, or feel objects, tools or controls

5. Regularly exposed to the risk of bloodborne diseases and other transmissible infections.

6. Communicates with others (including patients) and must effectively must be able to exchange information accurately via phone, email, and or in person.

7. Specific vision abilities apply including close vision, distance vision and ability to adjust focus.

8. Subjected to varying and unpredictable situations, including weather conditions.

9. Subjected to irregular hours.

10. May have contact with hazardous materials.
